SOLUTION FOR INCOMPETENT CERVIX.
✅The primary solution for cervical incompetence (cervical insufficiency) is a cervical cerclage, a surgical procedure where a stitch is placed around the cervix to keep it closed, usually between 11 and 24 weeks of pregnancy.
✅Other treatments include progesterone supplements, regular ultrasound monitoring, and a va**nal pessary to support the cervix.
Key Treatment Options:
🔴Cervical Cerclage (Stitch):
Vaginal Cerclage (McDonald or Shirodkar technique): The most common method, performed through the va**na to reinforce the cervix.
🥚Transabdominal Cerclage: Used if a va**nal cerclage is not possible or has failed previously; the stitch is placed higher up at the junction of the uterus, often done laparoscopically.
🥚Timing: Usually planned between 11–24 weeks, though emergency cerclage can be done later.
🥚Removal: Stitches are typically removed around 36–37 weeks of pregnancy to allow for labor.
🔴Progesterone Supplementation: Vaginal progesterone (pessary or gel) is used to relax the womb and lower the risk of preterm labor, often continued until 34 weeks.
🔴Ultrasound Monitoring: Frequent transva**nal ultrasounds (usually every 1-2 weeks) to check for premature cervical shortening or funneling (opening).
🔴Pessary: A silicone device inserted into the va**na to support the cervix, sometimes used in addition to or instead of a cerclage.
🔴Lifestyle Adjustments: While strict bed rest is generally not recommended, reducing physical activity and avoiding heavy lifting is advised.
When to Seek Care:
✅If you have a history of second-trimester miscarriages or a known weak cervix, early, regular prenatal checkups are crucial to determine the best preventative, often called a prophylactic, cerclage.
Contraindications:
✅Cerclage is not recommended if there are signs of active preterm labor, infection (intrauterine infection), or premature rupture of membranes (PPROM).
