Here’s what most people are never told before a sleeve: the sleeve is a high-pressure tube. That pressure has to go somewhere — and in a patient with a hiatal hernia or existing reflux, it goes up. Into the esophagus.

Her endoscopy showed esophagitis. Damaged tissue. Not a diet problem. An anatomy problem.

So we converted her sleeve to a bypass. The bypass diverts acid away from the esophagus entirely — it’s the operation that actually treats reflux, not just masks it.

She’s off the medication. She sleeps flat.

If you had a sleeve and you’re taking acid reducers every day: that is not normal, and it doesn’t get better on its own. Reflux left untreated damages the esophagus over years.

Most people think the next step after losing weight is plastic surgery… but that’s not always true.

After treating thousands of patients, I’ve learned that great results don’t depend only on the surgeon.

They depend on getting the timing right.

If you’re considering plastic surgery after bariatric surgery, here are five things you should never overlook.

First: Wait at least 12 to 18 months. Your body is still changing.

Second: Make sure your weight has stabilized. Having surgery while you’re still losing weight can affect your final results.

Third: Check your hemoglobin. After bariatric surgery, anemia is more common than most people realize.

Fourth: Prioritize protein. It doesn’t just help preserve muscle—it plays a critical role in proper healing and recovery.

And the fifth tip…

It’s the one that often makes the biggest difference between a good result and a truly lasting one.

Almost no one talks about it.

Planning bariatric surgery? Your safest recovery starts before the operation. Stop smoking for 3 months, avoid alcohol for 30 days, and discontinue cannabis and NSAIDs as directed by your surgeon. Save this guide for later and comment “CHECKLIST” if you’re preparing for weight loss surgery.

You’ve been told to eat less. Move more. Try harder.

But if you have PCOS, type 2 diabetes, high blood pressure, or sleep apnea — I can tell you after 15,000+ surgeries, willpower was never the problem. It’s metabolic. Your hormones and your weight are working against each other.

That’s the real difference between weight loss surgery and metabolic surgery. One shrinks your stomach. The other resets the system that’s been fighting you — blood sugar, blood pressure, fertility, sleep.

I just got back from three weeks off, and this was the first surgery of the season. Every time I’m in the OR I’m reminded this isn’t cosmetic — it’s giving people their health back.
