Honoured to have completed the EndoSpine Academy 2026 Advanced Course at Balgrist University Hospital, Switzerland.

16 SGNC CME points
(SGNC - Schweizerische Gesellschaft für Neurochirurgie aka Swiss Society of Neurosurgery)

This unique program brought together 8 world-renowned masters in endoscopic spine surgery and only 12 selected participants from around the world, creating an exceptional environment for learning, discussion, and collaboration.

The cadaveric training, case discussions, and personal interactions with both faculty and fellow surgeons made this an invaluable experience.
Special thanks to Professor Mazda Farshad, Professor Luke Kim, Prof Dem, and the entire faculty for their generosity in sharing their knowledge and expertise.

Grateful for the opportunity to learn from the best, and excited to bring these insights back to benefit my patients and further advance endoscopic spine surgery in Malaysia.

1 tahun kelemahan kaki… hingga kursi roda dan terbaring.”

Seorang pasien muda mengalami kelemahan kedua kaki yang semakin memburuk selama 1 tahun.

Awalnya masih bisa berjalan.
Kemudian harus menggunakan kursi roda.
Akhirnya menjadi bed-bound.

Setelah berobat di negaranya, beliau datang dari negara tetangga untuk mendapatkan penanganan lebih lanjut.

Pemeriksaan menunjukkan penekanan saraf berat pada tulang belakang cervical

Dilakukan operasi buang tumor saraf menggunakan teknik mikroskopik untuk melepaskan saraf yang terjepit.

✨ Hari ke-2 pasca operasi:
Beliau sudah dapat mengangkat kedua kaki dengan kuat.

Pemulihan masih berjalan, namun perbaikan dini ini memberi harapan besar untuk kembali berjalan.

This is a. Large L5/S1 disc herniation with a sequestered fragment.

In this case, a sizable disc fragment (approximately 3 cm) was compressing the nerve root.
Using an endoscope, the disc was removed through a small incision with minimal disruption to surrounding muscles and tissues.

The video shows:
• Pre-operative MRI findings
• Endoscopic removal of the disc fragment
• Retrieved disc specimen
• Small skin incision after the procedure

Endoscopic spine surgery allows targeted nerve decompression while preserving normal anatomy in appropriately selected patients.

A landmark study published in JAMA Surgery (Epstein et al.) showed that surgeons suffer very high rates of neck, back, shoulder, and arm pain—comparable to, and in some aspects even higher than physically demanding occupations such as construction work.

Surgery may look “clean” and “controlled,” but the physical strain is real.
Long hours, static posture, repetitive movements, and operating with loupes, microscope and endoscope all take a toll on the surgeon’s body.

📱 Handphone Habit, Neck Pain Later?

Many of us spend hours looking down at our phones without realising the impact on the cervical spine.

When the head is tilted forward 30–60 degrees, the weight on the neck increases from the normal 4–5 kg to as much as 25-30 kg — imagine carrying a small child on your neck every day.

A recent study on healthy adults shows that prolonged smartphone use can:
• Change cervical alignment
• Increase muscle strain
• Lead to chronic neck pain and headaches
• Accelerate wear-and-tear in the spine

Good news: Small adjustments can protect your spine:

✔ Bring the phone up, don’t bring your head down
✔ Take breaks every 20–30 minutes
✔ Strengthen deep neck flexors
✔ Mind your sitting posture at work and home

Neck pain is common, but it’s not “normal.”
If you or someone you know has persistent neck pain, numbness, or arm tingling — seek proper assessment early.

Your spine only has one job: support you through life.
Take care of it.

Many struggle with claudication, leg pain, cramp or numbness when walking — often caused by lumbar spinal stenosis.

At ThaiSMISST 2025, I had the chance to share one of the most important updates in EndoSpine surgery:
