【 #失智專題】難以普及…失智針劑太貴 僅500多人施打

國內失智症患者近四十萬人，去年兩款可減緩失智惡化的針劑上市，可清除沉積於大腦組織的類澱粉蛋白，但臨床醫師估計，全台僅五、六百名早期阿茲海默症的失智個案接受針劑治療，主因在於費用昂貴，每個療程至少一五○萬元。

成大醫院失智症中心主任白明奇說，新藥問世後，臨床使用生物標記診斷技術也隨之精進，失智症進入精準診斷，邁進阿茲海默症精準診斷的新紀元，「從事失智的診斷、治療及照護已廿、卅年，從來沒有像現在如此充滿希望」。

亞東醫院神經醫學部失智中心主任甄瑞興表示，失智新藥為免疫療法，可有效清除大腦內類澱粉蛋白沉積，宛如清除大腦組織內的雜質、垃圾、毒素，進而延緩失智。愈早治療，療效愈好，能夠改善失智引起的情緒及思考，避免惡化。

中國醫藥大學附設醫院失智醫療中心副主任盧韻如表示，新藥上市後，國人對失智症的病識感明顯提升，以前門診患者常因多次走失、幻聽幻覺，家人才陪同就醫。但現在不少人只是自覺記憶不好、忘東忘西，擔心自己可能罹患阿茲海默症，而主動檢查。

相較於臨床醫師對於新藥的樂觀，病患及家屬的反應卻不如預期，以中國醫藥大學附設醫院為例，一一四年八月至今年六月底，新確診失智症患者達九七三人，但實際接受新藥治療人數卻少之又少，迄今僅十五人。雙和醫院則有卅名患者接受新藥治療，年齡最輕者六十歲，最高者年逾八十。

目前以成大醫院收治新藥注射患者人數最多，共八十九人，女性占七成、男性三成，平均年齡七十一歲。亞東醫院去年六月替阿茲海默症病人施打新藥，至今累積治療卅五人，男性十三人、女性廿二人。

雙和醫院失智症中心主任黃立楷表示，針劑新藥如要普及，需克服兩道關卡，一為需接受「類澱粉蛋白正子攝影」（ＰＥＴ）或腦脊髓液等檢查，用藥前評估即先花上數萬元，再者，目前一個療程需一五○萬至一八○萬元，加上治療期間仍有「類澱粉蛋白相關影像異常」（ＡＲＩＡ）等風險，以致新藥收治人數不如預期。

白明奇建議，衛福部針對帶有APOE4特殊基因，且已有失智症狀、符合失智好發年齡等高風險族群，補助抽血篩檢費用，讓失智患者願意踏出針劑治療的第一步。一旦血液檢測報告中的「濤蛋白」數值超過標準值，則表示大腦類澱粉沉積程度嚴重，即可安排正子攝影，了解沉積情形，評估用藥。
