2-Years Follow-Up & Remaking of an Orbital Prosthesis
1. Background & Clinical Presentation
Prosthesis Lifespan: ⏱️
Silicone orbital prosthesis typically have a lifecycle of 2–3 years. Longevity depends heavily on wear frequency, UV radiation exposure, environmental factors, cleaning hygiene, and the patient's technique in applying and removing medical-grade adhesive.
Patient Status:
After 2 years of continuous wear, the patient presented with prosthesis degradation, including discoloration, tearing of the silicone borders, and staining around the adhesive margins.
Patient Goal:
The patient requested a replacement prosthesis with a more natural aesthetic that is easier to maintain.
2. Technical Challenges & Fabrication Failure ⚠️
Material Fault:
During the initial processing of the new silicone batch, a curing failure occurred.
Resulting Defects:
The silicone developed a porous and spongy texture with internal micropores.
Clinical Implications:
Washing the defective prosthesis caused water entrapment within the micropores. Because the medical adhesive is hydrophobic, trapped water leaking out caused spontaneous debonding and adhesive failure.
Outcome:
Despite achieving an excellent intrinsic skin tone match, this batch had to be rejected due to compromised structural integrity.
