Case of Impacted Maxillary Canine🦷

This patient was referred from other dental clinic for management of impacted upper left maxillary canine. The canine tooth was failed to erupt and trapped in the bone.

After a detailed examination and evaluation, we decided on orthodontic traction with gold chain to save her impacted maxillary canine.

With careful planning and correct mechanic, the impacted maxillary canine has been successfully bring down to the dental arch.

What is impacted maxillary canine?
An impacted maxillary canine is a permanent upper canine that fails to erupt normally and remains trapped in the bone or soft tissue.

Common features:
* More common in females
* Usually palatal
* Retained primary canine
* May cause root resorption of adjacent teeth

Management:
* Orthodontic space creation
* Surgical exposure + orthodontic traction

Dental implants are artificial tooth roots made of titanium that are surgically placed into the jawbone to replace missing teeth. A crown is then attached on top so it looks and works like a natural tooth.

General process:
1. Consultation + X-ray/CT scan: Dentist checks if your jawbone is thick enough.
2. Implant surgery: Titanium screw is placed into the bone. Then you wait 3-6 months for “osseointegration” = bone fuses with the screw.
3. Attach abutment + crown: Once it’s solid, the crown is placed on top.

Pros:
- Looks and feels like a natural tooth
- No need to grind down neighboring teeth like with a bridge
- Long-lasting, can be 15-20+ years if cared for properly
- Helps prevent jawbone shrinkage

An impacted lower premolar is a premolar tooth in the lower jaw (mandible) that fails to erupt into its normal position.

Common causes:
1. Lack of space in the dental arch.
2. Early loss or prolonged retention of the primary (baby) tooth.
3. Abnormal tooth angulation or position.
4. Obstruction by adjacent teeth, cysts, or bone.

Clinical features
- Often asymptomatic
- Delayed eruption or missing premolar
- Retained primary molar
- Possible pain, swelling, or infection if pathology develops

Radiographic findings
- Tooth located below the occlusal plane
- May be mesio-angular, disto-angular, vertical, or horizontal
- Possible associated radiolucency (e.g. follicle enlargement, cyst)

Management
-Observation if asymptomatic and no pathology
-Surgical exposure with orthodontic traction
-Extraction if unfavourable position or pathology present

An anterior open bite is a type of malocclusion where the upper and lower front teeth do not touch when the back teeth are together, leaving a vertical gap between them.

🦷 Types of Anterior Open Bite

1. Dental open bite

-Caused primarily by the position of teeth.
-Teeth fail to erupt properly in the anterior region.
-Often linked to habits like thumb sucking or tongue thrusting.

2. Skeletal open bite

-Caused by jaw growth discrepancies.
-The upper and/or lower jaw grows in a way that prevents front teeth from contacting.
-Often associated with long face syndrome.

⚠ Causes

-Oral habits: thumb sucking, pacifier use, tongue thrusting.
-Skeletal growth problems: excessive vertical growth of the jaw.

What is a Protraction Facemask?

The device commonly referred to as a protraction facemask (also called a reverse-pull headgear or orthodontic facemask) is an extra-oral orthodontic appliance used primarily in growing children.

Purpose

It’s used when the upper jaw (maxilla) is under-developed or positioned too far back relative to the lower jaw (mandible) — essentially in a skeletal Class III malocclusion.

The facemask applies forward and downward traction on the maxilla (via elastics hooked to a fixed intra-oral appliance) to encourage maxillary advancement.

By doing this early, the hope is to reduce the severity of the discrepancy and possibly reduce the need for more invasive treatment (e.g., jaw surgery) later.

What it Can (and Cannot) Do

Can advance the maxilla forward (modestly) and help correct or reduce the underbite/crossbite.

Improve dental occlusion (overjet, overbite) in growing patients.
