PHYSICAL ACTIVITY FOR ELDERLY
老年人身体活动

As people age, they commonly experience an increase in frailty and physical weakness. This decline in physical function often results in a heightened risk of injuries, particularly due to compromised balance and coordination. Such changes make older adults more vulnerable to falls, which can lead to significant health complications and further loss of independence. However, these risks can be prevented by being physically active and through healthy eating.
随着年龄的增长,人们常出现虚弱及体能衰退的现象。身体功能的下降,尤其是平衡能力与协调性的减弱,会显著提高受伤风险,使老年人更容易发生跌倒。跌倒不仅可能引发严重的健康问题,还会进一步影响其日常生活能力与独立性。然而,通过维持规律的身体活动及均衡的健康饮食,这些风险在很大的程度上是可以预防的。

Previously, we have discussed on the healthy eating component. In this article, we will address on the physical activity for elderly. Why is physical activity crucial for them? Aside from preventing frailty and physical weakness through improvement of muscle mass, muscle strength, bone strength, balance and coordination, research shows that physical activity can protect against noncommunicable diseases such as heart disease, diabetes, stroke and high blood pressure. It also helps in delaying the onset of dementia, preventing cancer, improving mental health, cognitive function, sleep and overall quality of life. Physical activity may also help in boosting the immune function. Meanwhile, physical inactivity in elderly people is associated with greater risk of chronic diseases, decrease in muscle mass and strength, and reduced quality of life.
在此前的内容中,我们已探讨了健康饮食的重要性。本文将进一步聚焦于老年人身体活动这一关键课题。那么,为什么身体活动对老年人如此重要?身体活动不仅有助于增加肌肉量、提升肌肉能力与骨骼强度,还能改善平衡能力与协调性,从而有效预防虚弱与体能衰退。研究显示,规律的身体活动可降低多种非传染性疾病的风险,如心脏病、糖尿病、中风及高血压。此外,它还有助于延缓失智症的发生、降低癌症风险,并改善心理健康、认知功能、睡眠质量以及整体生活质量。同时,身体活动也可能在一定程度上增强免疫功能。相反,缺乏身体活动则与较高的慢性疾病风险、肌肉量和肌力下降,以及生活质量降低密切相关。

How much physical activity do older people need? Any types of physical activity including light activities during leisure time is more beneficial than none. Guidelines generally recommend elderly to do similar amount of physical activity as adults below 60 years old, which is at least 150 minutes of moderate, or 75 minutes of vigorous intensity aerobic exercise per week, or a combination of both. Additionally, they are also recommended to incorporate strength, flexibility and balance exercises as they have additional benefits for elderly.
老年人需要多少身体活动量呢?事实上,任何形式的身体活动,包括休闲时间中的轻度活动都比完全不活动更有益于健康。一般建议,老年人的身体活动量应与60岁以下成年人相近,即每周至少进行150分钟的中等强度有氧运动,或75分钟的高强度有氧运动,或两者的组合。此外,老年人也应结合进行肌力、柔韧性及平衡训练,因为这些运动能带来额外的健康益处,有助于维持功能并降低跌倒风险。

Aerobic exercise:

Aerobic exercise helps strengthen the heart and lungs and increase fitness.
有氧运动有助于增强心肺功能,提高身体耐力与整体体能水平。

- Moderate /中等强度有氧运动
Elderly should aim for 150 minutes of moderate aerobic activity per week, for instance, 30 minutes for five times a week. This can be separated into smaller sessions if needed (for instance, 10 minutes, 3 times a day). Examples of activities include walking, cycling, swimming and dancing. Mixing up activities is recommended to prevent overuse of muscles. Moderate intensity activities should be making you feel a bit breathless and starts sweating but is still able to speak comfortably.
老年人每周应进行约150分钟的中等强度有氧运动,例如每周进行5次、每次30分钟的活动。若有需要,也可以将运动时间分散为较短的时段完成,例如每天3次、每次10分钟。
常见的运动形式包括步行、骑自行车、游泳及跳舞等。建议多样化运动方式,以避免同一肌群的过度使用。
在中等强度运动下,身体会略微感到气喘并开始出汗,但仍能较为轻松地进行对话。

- Vigorous /高强度有氧运动
If elderly prefer vigorous activities, 75 minutes of exercise per week should be adequate. Examples of activities include jogging or running, fast cycling, swimming laps and high-intensity dancing such as Zumba. Vigorous activities should be making them feel difficult to hold a conversation.
若老年人选择进行高强度身体活动,建议每周约75分钟即可。常见活动包括慢跑或跑步、快速骑自行车、游泳(来回游)、以及高强度舞蹈(如Zumba)等。
在高强度运动过程中,身体会明显感到吃力,呼吸加快,且通常难以在运动时顺畅交谈。

Strength/Resistance:
肌力/抗阻训练:

Strength exercise helps increase muscle and bone strength, consequentially reducing the risk of fall. Equipment such as weights, resistant bands and even household items like water bottle can be used. Strength exercise can also be done using body weight such as sit up, half squad, plank and push up. One should aim for twice per week of strength exercise, with at least 48 hours of break in between, and start with lighter weight to prevent muscle overuse and injury. Typically, one should achieve 10 to 15 repetitions for each major muscle group (legs, hips, back, abdomen, chest, shoulders, and arms). Repetition is a full range of movement, for instance bending and straightening of arms during push up exercise.
肌力训练有助于增强肌肉与骨骼强度,从而降低跌倒风险。可使用多种器材进行训练,例如哑铃、阻力带,甚至水瓶等日常物品;同时也可以利用自身重量进行训练,如仰卧起坐、半蹲、平板支撑及俯卧撑等。
建议每周进行至少两次肌力训练,并在两次训练之间至少间隔48小时。初学者应从较轻的负重开始,以避免肌肉过度使用及受伤。
在训练过程中,每个主要肌群(包括腿部、臀部、背部、腹部、胸部、肩部及手臂)通常建议完成约10至15次重复动作。所谓“重复”,是指一个完整的动作范围,例如在俯卧撑中,手臂的弯曲与伸直即为一次重复。

Flexibility:
柔韧性训练:

Flexibility exercises include stretching and bending, which can prevent stiffness of muscles and joints and improve range of movement. These activities can be done as a warmup before other exercises or even during leisure times such as while watching the television. One should do flexibility exercise at least 10 minutes, twice per week. When stretching, try to hold the stretch for 10 to 30 seconds for effectiveness.
柔韧性训练包括伸展与弯曲动作,有助于预防肌肉与关节僵硬,并改善身体的活动范围。这类运动可作为其他运动前的热身,也可在休闲时间进行,例如在观看电视时完成简单的拉伸。
建议每周至少进行两次柔韧性训练,每次约10分钟。在进行伸展时,每个动作应保持10至30秒,以达到较好的训练效果。

Balance:
平衡训练:

Balance exercises such as heel-to-toe walking, sideways walking, single leg stand, tai chi and taking the stairs helps to enhance stability of posture, increase brain-to-muscle coordination and improve core and lower-body muscle strength. These can subsequently reduce the risk of fall and aids in maintaining mobility. It is recommended to do balance exercises for twice to three times a week.
平衡训练如脚跟对脚尖行走、侧向行走、单脚站立、太极及上下楼梯等有助于增强身体姿势的稳定性,提升大脑与肌肉之间的协调能力,并加强核心肌群及下肢力量。这些改善有助于降低跌倒风险,并维持良好的活动能力。
建议每周进行2至3次平衡训练,以获得更佳效果。

Any physical activities should be done according to the elderly’s level of fitness to prevent injuries. Beginners should start with small amount of physical activity and gradually increase the intensity, duration and frequency of the exercise routine over time. Seeking advice from healthcare professionals are recommended especially for those with underlying medical conditions such as those with history of falls or is at high risk of falling and those who want to start with vigorous activities.
所有身体活动都应根据老年人的体能状况量力而行,以预防运动损伤。初学者应从少量、低强度的身体活动开始,并随着时间逐步增加运动的强度、时长与频率。
对于患有基础疾病的人群,例如有跌倒史或跌倒风险较高者,以及希望开始进行高强度运动的人士,建议在开始运动前咨询医疗专业人员的意见,以确保运动的安全性与适宜性。

As previously discussed, fish and seafood are great for our health as they provide us with essential proteins, vitamins, and minerals with additional cons of reducing risk of cardiovascular diseases due to the high omega-3 and low saturated fat contents. With all these benefits, are there any negative health consequences to consumption of fish and seafood?
如前所述,鱼类及海产品因富含优质蛋白质、维生素和多种必需矿物质,而被认为对人体健康具有重要益处。此外,其高含量的ω-3脂肪酸及较低的饱和脂肪水平,有助于降低心血管疾病的发生风险。然而,在上述健康益处之外,鱼类及海产品的摄入是否可能带来潜在的不利健康影响,仍值得进一步探讨。

1) Contaminants
污染物

While fish and seafood provide many potential health benefits, they may be subjected to contamination with environmental toxic materials, which in turn may jeopardize human health. Among the common contaminants include metal such as mercury and lead, chemical such as pesticides and Polycyclic Aromatic Hydrocarbons (PAHs), physical such as microplastic, and others such as antibiotics. So, is it possible to obtain the benefits of fish and seafood consumption while avoiding harmful contaminants?
尽管鱼类及海产品具有多方面的潜在健康益处,但其可能受到环境中有毒物质的污染,从而对人体健康构成威胁。常见的污染物包括金属类污染物(如汞和铅)、化学类污染物,如农药和多环芳烃(Polycyclic Aromatic Hydrocarbons,PAHs)、物理性污染物,如微塑料,以及其他污染物,如抗生素。因此,在享受鱼类及海产品所带来的健康益处的同时,是否有可能有效避免这些有害污染物,成为一个值得关注的问题。

To reduce exposure to contaminants and subsequent health risk, some countries such as the USA and UK provide advisories and guidelines on the consumption of fish. Malaysia, however, does not have any specific guidelines. According to research, the commonly consumed fish and seafood in Malaysia are safe for consumption within normal dietary patterns, as heavy metal levels such as mercury, lead and cadmium typically fall below the maximum permitted levels set by the Malaysian Food Regulation (1985) and the Joint FAO/WHO Expert Committee on Food Additives (JECFA). Meanwhile, according to the WHO and FDA, scientific evidence shows that currently the level of microplastics or nanoplastics contamination pose no significant risk to human health. Thus, daily consumption of fish and seafood in moderation is safe.
为降低污染物暴露及其潜在的健康风险,一些国家,如美国和英国已针对鱼类消费制定了相关的建议与指南。然而,马来西亚目前尚未出台具体的鱼类与海产品摄入指南。根据现有研究,马来西亚常见食用的鱼类及海产品在正常膳食模式下是安全的,其重金属,如汞、铅和镉含量通常低于《马来西亚食品法规(1985)》以及联合国粮农组织/世界卫生组织食品添加剂联合专家委员会(JECFA)所规定的最高允许限值。此外,世界卫生组织(WHO)和美国食品药品监督管理局(FDA)指出,现有科学证据表明,目前微塑料或纳米塑料的污染水平尚未对人体健康构成显著风险。因此,在适量摄入的前提下,日常食用鱼类及海产品被认为是安全的。

Even so, to be safe, here are a few tips that may help mitigating the effect of contaminants when fish and seafood are consumed:
即便如此,为进一步保障安全,以下建议或可在食用鱼类及海产品时帮助降低污染物可能带来的影响:

- Remove the fat part of the fish by cutting them off or melting it off during cooking. Some contaminants like PCBs and pesticides can be reduced through this method. Heavy metals however are mostly bound to the meat of fish, therefore will not be removed through removing the fat.
通过切除鱼体脂肪含量较高的部位,或在烹调过程中使脂肪融出,可在一定程度上减少污染物的摄入。部分污染物,如多氯联苯〔PCBs〕和农药可通过该方式降低含量。然而,重金属通常主要与鱼肉组织结合,因此单纯去除脂肪并不能有效减少其含量。

- Eat smaller and non-predator fish such as Indian mackerel (kembung), sardine and pomfret (bawal) more often as they accumulate lower amounts of contaminants.
尽量多食用体型较小或非掠食性鱼类,例如印度鲭鱼(Kembung)、沙丁鱼和鲳鱼(Bawal),因为此类鱼类体内积累的污染物相对较少。

2) Cholesterol
胆固醇

Most fish are low in saturated fat in comparison to other meats. Certain shellfish and other seafood such as squid and prawn have high cholesterol content but are low in saturated fat. According to research, consumption of fish and seafood can reduce bad cholesterol in blood (non-HDL-cholesterol and LDL-cholesterol), increase good cholesterol in blood (HDL-cholesterol) and is linked to lower risk of heart disease. However, the American Heart Association US (AHA US 2019) advises that healthy dietary patterns such as Mediterranean and DASH diets that are inherently low in cholesterol should be followed, and emphasize should be put on consumption of fruits, vegetables, wholegrains, low fat or fat-free dairy, lean protein, nuts, seeds and liquid non-tropical vegetable oils. Furthermore, AHA US also a suggested that a serving of seafood should be about 85 grams (about 8-12 medium pieces or 3-4 pieces of large shrimp), and that seafood can be included in heart-healthy diet when paired with other lean or plant-based protein sources.

3) Uric Acid

Uric acid is normally produced by the body as the body breaks down a substance called purine. Typically, uric acid dissolves in the blood, are filtered by the kidneys and removed in the urine. However, excessive uric acid can cause formation of crystal stones, leading to gouty arthritis (crystal formation at joints) and kidney stones. Research shows that excessive consumption of seafood is associated with high uric acid level and increased risk of gouty arthritis. This is as certain seafoods are high in purine and can cause high uric acid in blood if consumed excessively. These seafoods include anchovies, mackerel, sardine, tuna, fish roe and shellfish. Some products made from seafood such as budu, belacan, cincalok and fish sauce are also high in purine. Therefore, fish and seafood should be consumed moderately along with other plant-based protein and lean meat.

In conclusion, fish and seafood can be valuable components of a healthy diet, offering important nutrients and potential health benefits. However, consumers should remain mindful of possible contaminants such as heavy metals, pesticides, and microplastics, and follow practical tips to minimise exposure. While scientific evidence supports the general safety of commonly consumed fish and seafood in Malaysia, moderation remains key, especially for individuals with specific health concerns such as cholesterol or uric acid levels. By making informed choices such as removing fatty parts and balancing seafood intake with other protein sources one can enjoy the benefits of fish and seafood while reducing associated risks.
总之,鱼类及海产品可作为健康饮食的重要组成部分,为人体提供丰富的营养素并带来潜在的健康益处。然而,消费者仍应关注可能存在的污染物,如重金属、农药及微塑料,并采取切实可行的措施以降低暴露风险。虽然科学证据表明,马来西亚常见食用的鱼类及海产品总体安全,但适量摄入仍至关重要,尤其对于血脂或尿酸水平有特定健康顾虑的人群。通过做出明智的选择,例如去除脂肪含量较高的部位,并将海产品摄入与其他蛋白质来源合理搭配,人们可以在享受鱼类及海产品营养与健康益处的同时,降低潜在的健康风险。

Fish and seafood is commonly consumed daily in Asian countries. Particularly in Malaysia, the population consumes at least one to one and a half medium fish once a day. According to the Malaysian Dietary Guidelines 2020, daily consumption of fish or seafood is important to achieve healthy and balanced diet while other guidelines recommended at least two serving of fish and seafood per week, one of which should be oily fish. This is as they provide many health benefits, such as protection from cardiovascular illness, support brain function, and anti-inflammation due to various nutrient contents like:
鱼类和海鲜在亚洲国家的日常饮食中占有重要地位。尤其在马来西亚,民众每天平均会摄取一条至一条半中等大小的鱼类。根据《马来西亚膳食指南2020》,每日食用鱼类或海鲜有助于达成健康且均衡的饮食;而其他膳食指南也建议每周至少摄取两份鱼类和海鲜,其中一份应为富含脂肪的鱼类。这是因为鱼类和海鲜富含多种营养素,能够带来诸多健康益处,例如预防心血管疾病、支持大脑功能及发挥抗炎作用,包括:

- Protein / 蛋白质
While fish and seafood contain the least amount of fat and saturated fat in comparison to meat and poultry, one serving, which is about a palm size contains similar amount of protein with meat and poultry. Protein is essential for bodily function such as immune system, structural growth and repair, enzyme and hormone production, and many more. Consumption of fish and seafood can ensure that adequate protein is obtained without increasing the risk of cardiovascular diseases due to high saturated fat consumption.
与肉类和家禽相比,鱼类和海鲜所含的总脂肪和饱和脂肪较低,但每一份约为手掌大小的鱼类或海鲜,其蛋白质含量却与肉类和家禽相当。蛋白质对人体多项生理功能至关重要,包括维持免疫系统运作、促进身体组织的生长与修复,以及参与酶和激素的合成等。适量摄取鱼类和海鲜,不仅有助于确保充足的蛋白质摄入,还能在不增加高饱和脂肪摄取的情况下,降低心血管疾病的风险。

Aside from having low saturated fat content, fish and seafood, especially oily fish such as mackerel (kembung), tuna, herring (parang) and torpedo scad (cencaru) are generally high in omega-3 PUFA (EPA and DHA). Omega-3s are crucial for structure of cell wall, cognitive function and foetal development. They are also beneficial in lowering risk of cardiovascular diseases and inflammation.

Vitamin E acts as antioxidants, preventing inflammation and strengthen the immune system, while vitamin A is needed for healthy eyesight, skin and tissue, immune system and reproduction.

Calcium is important for bone and teeth, muscle and nerve functions, while iodine is needed for thyroid function that is responsible for metabolism and growth. Selenium acts as antioxidant and is also crucial for thyroid hormone metabolism, meanwhile phosphorus is required for formation of bones and teeth, as well as energy production. Iron primarily aids in blood cells production, while zinc supports the immune system, wound healing and growth.
