Here are 2 couples, both successful in their 1st attempts at IUI.

IUI has significantly lower success rates compared to IVF, hence it's not the ideal fertility treatment option for most couples. Especially in those with bilateral tubal or severe s***m issues.

IUI's good points are that they're less costly and are minimally invasive. Bad points, as mentioned, are their relatively lower success rates compared to IVF.

Problems:
Recurrent miscarriages
Recurrent implantation failures
Thyroid disorder
PMOS
S***m issues

1st Ivf, 11 blastocysts, non PGT tested.
● 1st frozen embryo transfer (FET) - abandoned due to suboptimal scan findings
● 2nd FET - miscarriage 8 weeks
● 3rd FET - failed
● 4th FET - biochemical pregnancy

She then saw me for a second opinion.

We decided to:
# Thaw her remaining 8 embryos for PGTA - we had 3 euploid and 1 mosaic embryo to use

# Do an endometrial receptivity analysis and microbiome study - result showed receptive lining but there was an ongoing infection

# Added immune modifiers to her next FET protocol

We transferred a euploid embryo and she's now 12 weeks pregnant. I've sent her back to her Obstetrician in Penang for further pregnancy care. Due date, Chinese New Year's eve, February 2027. (No confinement lady, not my problem 😆).

As she left, she said 'See you soon'.
No way! Dont come back anytime soon, Mum! The earliest you can come back to see me, is in 1 and a half years from now, for baby Number 2 🤣!

Married 14 years, low AMH 0.28ng/ml.
1st embryo transfer failed.
2nd embryo transfer - 1 embryo has split into 2 and made twins! Such awesome luck ❤️, for someone with very low ovarian reserve.
Plus, we dont have any spare embryos.
Needless to say, couple is over the moon!
Due date February 2027.

So worth it! All the anxiety, trepidation, profound feelings of loss of control and isolation - all gone, with the birth of this beautiful baby girl.

PCOS/PMOS patients generally have high success rates with fertility treatment, especially those ladies who have irregular menstrual cycles.

Married in 2022, this 28 years old Mum has PMOS with me**es every 3 months once, meaning she doesn't ovulate regularly. Her AMH is high, 54 pmol/L. Husband’s s***m test is normal.

I advised them to start with very simple fertility treatment, and forget IVF for now.

It took us only 2 ovulation induction cycles with letrozole to get them pregnant!

They are 8 weeks pregnant now and they're expecting their baby by February 2027.

The moral of the story is that, IVF doesn't have to be the solution in each and every scenario.

Both these ladies had their embryo transfers on the 20th April 2026 with me at Sunfert@BangsarSouth. Both are 12 weeks + 1 day pregnant today. And so, both have the same due date, 6th January 2027. Both ladies also had failed embryo transfers with their previous Doctors, before coming to see me.

Both ladies had their final visit with me today, for this current pregnancy. Ladies, I wonder if your kids will ever cross paths again, somewhere in their futures.

Recurrent embryo transfer failures with 3 blastocysts and recurrent miscarriages twice.
She then switched Fertility Specialists and came to see me at Sunfert@Bangsar South. She still had 8 frozen embryos.

Mum is a strong willed 35 year old woman from Penang. She makes day trips for our consultations, Penang - KL - Penang.

My advice to her was:
1. Thaw the remaining embryos and do PGTA
2. Endometrial receptivity analysis
3. Immune analysis
4. Treat her thyroid hormone abnormality, even though it was only mildly abnormal
5. Add immune-modifiers in her frozen embryo transfer (FET) protocol
6. Use higher doses of estrogen pills since she had a suboptimal endometrial lining

We transferred one euploid embryo in May 2026.

Result: Positive pregnancy test after her 1st FET with me.
Currently 7 weeks pregnant.
Due date: February 2027.

14 years wait, for these twins. Married 2012, Mum's 31 years old. She has Premature Ovarian Insufficiency, her AMH is only 2pmol/L. She will likely go into Premature Menopause soon.

You can see how low her AMH is on the graph below.

■ We did 1 IVF cycle, collected 5 eggs and she made 3 blastocysts
■ Her 1st frozen embryo transfer (FET) failed
■ Her 2nd FET has resulted in an embryo which has split into 2, so she's now pregnant at 7 weeks with identical twins.

Isn't that just extremely lucky? Her due date is in February 2027. Hope all goes well for Mum and her twins!

If you're planning to delay childbearing, at the very least, do an AMH to see what your ovarian reserve looks like.
