One of my nurses told me this week that her kids said, "It's Dr Alex again. He always wants to start cases so early." She laughed. I laughed. Then it sat with me.
When a procedure goes smoothly. When patients tell me the nurses were kind, the team was warm, everything just worked, that isn't luck. It's a group of people who have given up part of their morning, their evening, sometimes their family time, so that your care runs the way it should.
This one is for them. The nurses. My anaesthetic colleagues. The endoscopy team. The porters, the cleaners, the assistants. The people who arrive before the lights come on and stay until the last patient is safe.
And it is also for the families at home, the kids who notice when their mum or dad leaves before breakfast who quietly carry part of the cost of this work too.
You're the reason any of this works. Thank you. 💙🙏

Three patients with severe acute pancreatitis admitted this week alone. It is one of the conditions I most wish people would take seriously sooner.
Gallstones, alcohol, and uncontrolled triglycerides are the main causes. The warning sign is severe upper tummy pain that radiates through to the back — usually after a fatty meal or heavy drinking — with vomiting that won't settle. That is not "gastric." That is an emergency.
The earlier you come in, the more we can do. Please share this — if one person recognises the pain and goes in early, this post will have done its job.

Why does a busy gastroenterologist spend his evenings making these videos?
Because the most valuable thing I do in clinic often isn't a procedure — it's helping a patient finally understand their own body. When that happens, the fear drops, treatment works better, symptoms settle, and patients learn to handle the next flare themselves — and to recognise when it's time to see a specialist.
In clinic I can only do that one patient at a time. Here, I can do it for all of you.
That's the whole point of this page: clear, honest gut and liver health from a real specialist — so you feel empowered, not frightened.
Follow along. Let's take the mystery out of your gut health, together. 💙

"Gastric" might be the most overused word in Malaysian clinics 😅 But reflux and dyspepsia are different problems with different treatments. Knowing which one you have changes everything. Save this for your next clinic visit.

Today, on the last reel of my World IBD Day 2026
series — one message worth holding onto.

This past week, we journeyed through IBD together:
what it is, why it's rising, and what truly changes
the outcome.

IBD is rising. But how it turns out is not set
in stone.

The earlier we act, the more we can change.

→ Find it early
→ Watch it closely
→ Treat it properly

If you have the symptoms — don't wait.
If you love someone who does — help them get checked.

That's how we turn IBD into a life fully lived.

Thank you for walking this week with us.
From Gut to Heart, this is only the beginning.

"Doctor, when can I stop my medication?"

I hear this every week from IBD patients.

The honest answer depends on three things and
most patients don't realise what they are.

→ What deep remission actually means
→ Why de-escalation strategy matters
→ Three groups who should never stop

Tap to watch. Send to anyone on long-term
IBD therapy.

[WORLD IBD DAY 2026]

🩺 5 things I hear from IBD patients.

Every one of them delays diagnosis — sometimes
by years.

❌ "It's just IBS"
❌ "I'm too young for Crohn's"
❌ "The blood will go away"
❌ "It's just stress"
❌ "I'll fix it with diet"

Tap to watch. Then send this to someone who
needs to hear it.

🌍 May 19th is World IBD Day.

This year's theme is "IBD Has No Borders: Access to Care."
Here's what that actually means in Malaysia.
Three things slow patients down. ⬇️

1️⃣ Diagnosis delay
2️⃣ Biologics access
3️⃣ The specialist gap

If you or someone you love has Crohn's or ulcerative colitis, access is a fight worth having. Share with someone who needs to know.
