👁️ “Why Can’t My Ophthalmologist Just Remove My Cataract?” -Dr Chua
It is a very reasonable question. A cataract is one of the commonest causes of blurred vision, and cataract surgery can often restore excellent sight. But a cataract may not be the only reason your vision is poor.
Before recommending surgery, your ophthalmologist needs to examine the **retina and especially the macula** — the small, highly specialised area at the back of the eye responsible for our sharp central vision.
👀Why is this important?
Because other eye diseases can cause blurred or distorted vision at the same time as a cataract. Examples include the conditions shown in these photographs:
🍀Macular hole — a small defect in the centre of the macula that can significantly affect central vision.
🍀 Retinal vein occlusion — blockage of a retinal vein, which can cause bleeding and swelling of the macula.
🍀 Age-related macular degeneration (AMD)
— damage to the macula that can reduce central vision.
🍀 Diabetic retinopathy— diabetes can damage retinal blood vessels and cause bleeding, leakage and macular swelling.
This is why your ophthalmologist may dilate your pupils, examine the back of the eye and sometimes perform an **OCT scan of the macula** before cataract surgery.
👀The important message
**Removing a cataract clears the cloudy lens, but it cannot repair a damaged retina or macula.**
If significant macular disease is already present, cataract surgery may still be worthwhile, but **the vision may not improve as much as expected from cataract surgery alone**. In some cases, the retinal condition needs treatment as well.
So all those examinations and tests are not unnecessary delays. They help your ophthalmologist answer one of the most important questions before surgery:
“If I remove this cataract, how much improvent in vision can this patient realistically expect?”
A good cataract assessment is therefore not simply about looking at the cataract. **It is about making sure the rest of the eye is capable of seeing well once the cataract is removed.
