A lady had left ear discharge, itchiness and reduced hearing for one month.

Examination under microscope showed a large amount of blackish fungal debris in the ear canal. Microscope-guided suction was done to clear the fungal debris.

After cleaning, an existing eardrum perforation was seen. The patient was not aware of the perforation, so water exposure without proper ear precautions likely contributed to repeated ear discharge and recurrent infection.

Take home message: if you have persistent ear discharge or a known eardrum perforation, always keep the ear dry and seek ENT assessment early.

A hidden fish bone deep in the throat caused severe pain on swallowing.
On endoscopic examination, the fish bone was found near the epiglottis. An infected ulcer could also be seen on the epiglottis, likely caused by repeated rubbing from the bone. The fish bone was removed safely in the clinic with the help of rigid endoscopy.

If throat pain persists after eating fish, do seek proper ENT assessment.

This scope video shows a severe left quinsy, also known as a peritonsillar abscess.

There is swelling around the left tonsil area, with pus seen from the left peritonsillar region. The swelling also extends lower down into the throat, with swelling around the left aryepiglottic fold.

Common symptoms include severe throat pain, fever, pain when swallowing, muffled “hot potato” voice, and sometimes difficulty opening the mouth.

This is more serious than a normal sore throat and may need urgent ENT treatment, antibiotics, and drainage.

Chronic nose block and snoring should not be ignored.

This is an endoscopic before-and-after view of septoplasty and turbinoplasty in a 50-year-old patient with long-standing nasal blockage and snoring.

Before surgery, the nasal airway was narrowed due to a left deviated nasal septum and enlarged inferior turbinate. After surgery, the breathing passage became wider, allowing better nasal airflow.

Not all snoring is caused by the nose, but chronic nasal obstruction can contribute to mouth breathing, poor sleep quality, and worsening snoring.

For persistent blocked nose, snoring, or poor sleep quality, consider an ENT assessment.

A 25-year-old lady presented with 8 days of blocked nose, post nasal drip, cough and headache.

Nasal endoscopy showed thick infected mucus and severe inflammation at the back of the nose, around the adenoid area. After endoscopic suction, the swollen and inflamed adenoid tissue could be seen more clearly.

Although adenoid problems are more common in children, acute adenoid infection can also occur in adults.

If blocked nose, thick mucus, post nasal drip, cough or headache persists, it is advisable to seek medical assessment.
