⚠️Nasolacrimal Duct Obstruction in Cats

Nasolacrimal duct flush is a common veterinary procedure to diagnose and treat a blocked tear duct (nasolacrimal duct) in cats. A blocked nasolacrimal duct usually will have signs like excessive tearing, which can cause dark tear stains on the face, or mucoid or mucopurulent eye discharge. Most often blockages are caused by prior infections like feline herpesvirus, which can lead to scarring and narrowing of the duct.

💉 The Procedure
In most cats, the procedure is performed under local anesthesia eye drops or general anesthesia. Firstly, a small, blunt is gently inserted into the upper lacrimal punctum. By applying gentle pressure, sterile saline is flushed through the duct to see if it drains into the nose, this is indicated by the cat swallowing or fluid appearing at the nostril.

🔍 What to Expect
While helpful, a nasolacrimal flush is often more diagnostic than curative. It helps locate the blockage and assess ductal health. A fluorescein dye test may be performed first to check for blockages. For persistent cases, advanced imaging like CT or MRI may be needed. If the blockage is due to simple debris, flushing might resolve it. However, if it’s due to scarring, the problem may persist or recur.

⚠️ Follow-up and Further Treatment
If flushing isn’t a permanent solution, other treatments such as antibiotic or anti-inflammatory eye drops are required. As a last resort, surgery may be needed to create a new tear drainage pathway. It’s always best to have a veterinarian perform this procedure, hence please consult vet for a proper diagnosis and treatment.

⚠️What is a Prep***c Urethrostomy (PPU) in cats?🐱

A PPU is a major salvage surgery for male cats when a standard Perineal Urethrostomy (PU) fails. The urethra is rerouted to a permanent opening on the lower abdomen, just above the p***c bone to prevent life-threatening urinary blockages.

🧐When is PPU needed?
PPU is done after failed PU surgery due to scarring or re-obstruction, severe trauma or strictures of the pelvic urethra or insufficient urethral length for a standard PU.

🤔What to expect post-surgery in cat?
Usually after PPU surgery, recovery phase typically requires 14 days of strict indoor confinement. Most cats will need to wear an Elizabethan collar (cone) for the full 14-day healing period to prevent self-trauma to the stoma. The cat will urinate from the stoma on their lower abdomen rather than the normal anatomical route

🏥What are the potential complications post surgery?
Compared to a standard PU, a PPU has a higher risk of post-operative complications. It requires diligent at-home care and frequent veterinary monitoring such as urine scald due to constant dripping on the inside of the thighs and urinary incontinence due to partial or permanent loss of bladder control. Cats might be at a higher risk of recurrent lower urinary tract infections. Also, the new stoma can narrow over time, possibly requiring revision surgery. Because this is a complex procedure, thorough post-operative care and managing the stoma are critical for the cat’s quality of life.
