Urinary tract Infection - infection of any part of the urinary system from the urethra to the kidneys. The common symptoms are fever, painful urination, frequency of urination, abdominal pain, bed wetting in a child who had previously achieved bladder control. It may be lower or upper UTI. Children with upper UTI are usually very TOXIC, with high grade fever, vomiting, flank pain. After the age of 1yr, UTI is commoner in the girls than boys. Predisposing factors include perineal infections, (perineal hygiene would prevent this) Phimosis (circumcision in males is highly recommended to prevent this), pinworm infestation, obstructive lesions, voiding dysfunction, congenital malformation of the Urinary tract. Please do not use antibiotics irrationally. Visit the doctor who would order certain tests both blood, urine, sometimes radiological and prescribe appropriate treatment. Take plenty of water. In Children, strict follow up is important with periodic urine examination and cultures. Renal scarring, hypertension and End stage renal failure are possible complications that can be avoided in childhood if you visit your doctor early and choose not to use antibiotics irrationally.

Enuresis- persistent bedwetting beyond an age at which normal bladder control should have been achieved. (less than 10% of children will be bedwetting by 5years). It can be Primary (child has never achieved control) or Secondary (child had control for sometime but started bedwetting again). It is commoner in the males and there may be a positive family history.
Causes include, recent stressful life events, urinary tract infections, Constipation (large f***l mass compressing bladder), Endocrine causes like Diabetes and night time seizures, mental retardation, congenital abnormalities of urinary tract, Improper bladder training (too early or stressful training). In some cases especially the primary type there may be no identifiable cause.
Enuresis can occur during the day, night or both.
What to do-------DO NOT BEAT THE CHILD!, Rather try to identify stress factor and look for remedy. Restrict fluid intake at night, wake child up at midnight to urinate. Reward child for dry days and nights (may work for some of my lazy friends who would rather "p*e" in bed than drag themselves up from sweet slumber to urinate. if all these fail, see your Pediatrician who may prescribe some drugs and offer referral to a child psychologist. Anyway, the good news is that even without treatment, almost all of them will overcome this problem before they become adolescents. Show plenty of Love and understanding. Help to boost the child s morale. All the Best.
