🦷 Oral Submucous Fibrosis (OSMF)
Definition:
A chronic, progressive, potentially malignant oral mucosal disorder characterized by juxta-epithelial inflammation and progressive fibrosis, leading to stiffness of oral mucosa and restricted mouth opening.

Areca nut / to***co exposure

🔹 ↑ Collagen synthesis
🔹 ↓ Collagen degradation

Excessive collagen deposition

Juxta-epithelial fibrosis

Hyalinization + reduced vascularity

Loss of mucosal elasticity

Trismus + restricted tongue movement

⚠️ Potential malignant transformation → Oral Squamous Cell Carcinoma
💊 Management
1. Stop causative habits — MOST IMPORTANT
Complete cessation of areca nut, gutkha, to***co & related products
Patient education + regular follow-up
2. Nutritional support
Correct nutritional deficiencies
Antioxidant-rich diet may be advised
3. Medical therapy
Intralesional corticosteroids in selected cases
Hyaluronidase + corticosteroid may improve early/intermediate disease
Other agents such as pentoxifylline may be considered in selected patients
4. Physiotherapy
Regular mouth-opening exercises
Jaw-stretching devices/exercises
Particularly important after surgical treatment
5. Surgical management Indicated in moderate–severe trismus or advanced fibrosis:
Fibrotomy / release of fibrous bands
Excision of fibrotic tissue when appropriate
Reconstruction with buccal fat pad, nasolabial flap, tongue flap, skin graft, etc., depending on the defect
⭐ Key Point
Early diagnosis + complete areca-nut cessation + mouth-opening exercises = cornerstone of OSMF management.
⚠️ Because OSMF is a potentially malignant disorder, patients require long-term oral examination and surveillance for suspicious lesions.
