How does a surgical research idea grow from an initial brainstorming session into a published international standard?

In the latest episode of Inside Surgery (Season 7, Episode 4), host Dr Alice Tsai sits down with Dr Amila Cizmic and Dr Monica Ortenzi to discuss their 2023 paper published in Surgical Endoscopy. Together, they explore the development of their international Delphi consensus regarding surgical quality assessment in minimally invasive total gastrectomy for gastric cancer.

The conversation covers the entire journey of the project, highlighting how it evolved from a concept at the EAES Research Sandpit into a successful international collaboration. The guests reflect on the practical realities of surgical research, including expert selection, questionnaire design, methodology choices, and the future applications of their framework—including the expanding role of AI in surgery.

How many cases does an institution need to master Robotic Pancreatoduodenectomy? 🤖🏥

We know individual surgeon learning curves, but what about the whole hospital? A new study reviewed 100 consecutive RPDs at a high-volume center to find the exact institutional learning curve using RA-CUSUM analysis.

Key findings:
🔹 The Threshold: The institutional learning curve was achieved at 44 cases, and stabilized at 76 cases.
🔹 Improving Metrics: After 76 cases, operative time and major complications dropped significantly!
🔹 RPD vs. OPD: RPD showed longer OR times but resulted in shorter hospital stays and comparable safety and oncologic outcomes to open surgery.

The bottom line:
Adopting robotic pancreatoduodenectomy is safe and feasible for high-volume centers. Once the 44-case learning curve is passed, patients benefit from shorter hospital stays with outcomes comparable to open surgery. 🚀
