It is always stressful for the entire team to operate on neonates. Among the most stressed ones are the Anesthesiologist. Half of my stress of operating on these babies are taken off by my expert team who puts them down to sleep with the hope of freedom from disease on waking up.

नवजात शिशुहरूको शल्यक्रिया गर्नु सम्पूर्ण टोलीका लागि सधैं तनावपूर्ण हुन्छ। यस क्रममा सबैभन्दा बढी तनाव महसुस गर्नेहरूमध्ये एनेस्थेसियोलोजिस्ट (बेहोस पार्ने विशेषज्ञ) पनि पर्छन्। यस्ता शिशुहरूको शल्यक्रिया गर्दा हुने मेरो आधाजसो तनाव मेरो विज्ञ टोलीले कम गरिदिन्छ; उनीहरूले शिशुहरूलाई निदाउने अवस्थामा पुर्‍याउँछन्, ताकि ब्युँझिँदा ती शिशुहरू रोगबाट मुक्त हुन सकून्।

It is quite challenging and nerve wracking to operate on neonates. It gives both the surgeon and parents sleepless nights. But all that effort makes sense when you see smile on the mother’s face. This photo collage is that of a child with meningoencephalocele, before and after surgery.

नवजात शिशुहरूको शल्यक्रिया गर्नु निकै चुनौतीपूर्ण र तनावपूर्ण काम हो। यसले शल्यचिकित्सक र अभिभावक दुवैको निद्रा हराउने अवस्था सिर्जना गर्छ। तर, जब आमाको अनुहारमा मुस्कान देखिन्छ, तब ती सबै प्रयास सार्थक भएको महसुस हुन्छ। यो तस्बिर-संगालो 'मेनिन्गोएन्सेफालोसिल' (meningoencephalocele) को समस्या भएका एक शिशुको शल्यक्रिया अघि र पछिको अवस्थाको हो।

It is satisfying when decision to do no intervention wins over plans to operate in children. This child with microcephaly and craniosynostosis was advised to wait and watch after discussing with experts of pediatric neurosurgery. At one year followup the kid has normal head size and shape and is at par with kids of his age except for subtle weakness of left hand grip.

बालबालिकामा शल्यक्रिया गर्ने योजनामाथि हस्तक्षेप नगर्ने निर्णयले जित्दा सन्तोषजनक हुन्छ। माइक्रोसेफली र क्रेनियोसिनोस्टोसिस भएको यो बच्चालाई बाल चिकित्सा न्यूरोसर्जरीका विशेषज्ञहरूसँग छलफल गरेपछि पर्खनुहोस् र हेर्नुहोस् भनेर सल्लाह दिइएको थियो। एक वर्षको फलोअपमा बच्चाको टाउकोको आकार सामान्य छ र बायाँ हातको पकडको सूक्ष्म कमजोरी बाहेक उसको विकास उसको उमेरका बच्चाहरू जस्तै छ।

One major challange in congenical brain cyst is to prevent hydrocephalus and need of permanent shunt. The CT scans displayed shows how ventricles have decreased in size after surgery in these two kids after treatment. This makes shunt unnecessary.

जन्मजात ब्रेन सिस्टमा एउटा प्रमुख चुनौती भनेको हाइड्रोसेफलसलाई र स्थायी रूपमा शन्टको आवश्यकता रोक्नु हो। प्रदर्शित सीटी स्क्यानले उपचार पछि यी दुई बच्चाहरूमा शल्यक्रिया पछि भेन्ट्रिकलको आकार घटेको देखाउँछ। यसले शन्टलाई अनावश्यक बनाउँछ।
