I am sharing these operative picture as a testimony of how far we have come in providing optimum, minimal invasive and safe spine surgery.
This gentle man was operated for disc several years back and presented with a recurrence causing severe claudication in both lower limbs for last several months. Not relieved by any kind of conservative treatment.
We did a MIS Endoscopic TLIF (Fusion surgery), where implant is placed between two vertebrae that allows their fusion eventually.
Performing this kind of surgery on a site operated before requires an immaculate anatomical orientation, precise dissection and utmost care to not cause further harm.

Best wishes, for a smooth recovery, to our favourite actor. It is very encouraging that celebrities are trusting Nepalese medical institutions.
Dr. Baburam Bhattarai, earlier, underwent a gastric tumor resection at Bir hospital by Prof. Subodh Adhikari. Similarly Former PM K.P. Oli was operated twice by Nepalese surgeons at TUTH.
I am sure this trust presented by respected actor Deepa Shree will stand as a milestone for Brain tumour awareness and marker of Nepalese neurosurgical standards.
Big appreciation goes to Dr. Basanta Panta for leading the path of Nepalese Neurosurgery, the way he has for the last few decades.

एक जना उपाद्दे बाउन बा हात धोएर आधुनिक चिकित्सा विरुद्ध लागेका रहेछन्। वहाँ का पोष्ट हेर्दा दुइटा कुरा स्पष्ट हुन्छ :
१- वहाँले एउटा Quackery health home को paid promotion को जिम्मा लिनु भएको छ।
२- या, वहाँ मानसिक, बौद्धिक रुपमा सामाजिक बहसका लागी योग्य हुनुहुन्न‌।
Crosspathy अर्थात् एउटा विग्यता लिएर अरु विषयमा clinical practice गर्न मिल्दै मिल्दैन।
उदाहरण: म अहिले बिगत ८ वर्ष देखि Brain and Spine मा विग्य को रुपमा कार्यरत छु। मेरो MBBS को degree मा Gynaecology(स्त्री रोग) मा distinction छ , अब ऐले मैले स्त्री रोगका विषयमा पनि clinic संचालन गरे तपाईं के भन्नुहुन्छ? हो.... जब एउटै , Allopathy बिधा भित्र त Crosspathy गर्नु लाई Quackery भनिन्छ भने अरु कुरै नगरौं।

जहाँ सम्म कुन चिकित्सा विधि सर्वोत्कृष्ट भन्ने कुरो, Paralysis, Brain Tumor, Cancer, Heart attack, etc अनुभव गरेका बिरामीलाई सोध्नु। अरुलाई 'Refer to Jhakri!'

The ancient technique seen in this old documentary is called "Trephination". It simply means cracking the skull open.

How does it save lives?
Well, it doesn't actually cure the disease, however, trephination does relieve the pressure off the underlying brain structures for a while. Hence person receiving these surgeries from the witch doctors survived for a variable period after the procedure. Arguably similar to modern day decompressive craniectomy.

ब्रेन ट्युमर कति खतरनाक हुन्छन् त? के ब्रेन ट्युमर भनेको मृत्यु कै पर्याय हो त? मलाइ निकै रमाइलो लागेको एउटा कथा भन्छु।

पछिल्लो महिना मलाई एक जना ८२ वर्ष कि आमा टाउको दुखेको देखाउन आउनु भो। कुरो बुझ्दै जाँदा २० वर्ष अघि ब्रेन ट्युमर को अपरेशन गर्नुभएको भन्ने कुरो निस्कियो, डा. देवकोटा सँग।
Document छ? भनेको - निकै जतनले राखेको Discharge paper देखाउनुभयो। २० वर्ष अघि नै आँखै बाहिर धकेलेको(proptosis), दुइटा नशा थिचेको (Optic /Trigeminal compression ) बडेमानको ट्युमर(Sphenoid wing meningioma) रहेछ। अपरेशन पछि अहिले सम्म सामान्य टाउको दुख्ने बाहेक अरु केही समस्या छैन।

यो एकल घटना होइन। यस्ता थुप्रै दृष्टान्त छन्। सानो तथा विशेष प्रकृति भएका कतिपय ट्युमर बिना अपरेशन पनि ठिक हुन्छन्।

समयमा पहिचान र सहि उपचार नै ब्रेन ट्युमर बाट बच्ने उपाय हो।

Dr. Gongal started Neurosurgery in Nepal back in the 1960s. CT scans and MRIs were not even invented then. He performed life saving head surgeries based on clinical judgement and X-ray skull. The same Neurosurgery unit was later developed as a full fledged National Neurosurgical Referral Center at Bir Hospital by his successor Prof. U.P. Devkota. A humble grounded personality, who never pursued limelight, was active in Neurosurgical services upto his late 80s. Thank you Gongal Sir for laying the first stones on the path that we follow so passionately now.
We will always remember you.
