Hemolysis, icterus, and lipemia (HIL) are the three major pre-analytical interferences that can compromise the accuracy of laboratory test results, causing falsely increased or decreased analyte values and potentially affecting clinical decision-making.

1. 𝗛𝗲𝗺𝗼𝗹𝘆𝘀𝗶𝘀
Rupture of red blood cells, releasing hemoglobin and intracellular contents into plasma or serum, giving the sample a pink to cherry-red appearance.

𝗖𝗼𝗺𝗺𝗼𝗻 𝗰𝗮𝘂𝘀𝗲𝘀:
⁃ Improper blood collection, handling, transport, or storage (most common)
⁃ Less commonly, in vivo hemolysis (e.g., hemolytic anemia, transfusion reactions)

𝗠𝗲𝗰𝗵𝗮𝗻𝗶𝘀𝗺 𝗼𝗳 𝗶𝗻𝘁𝗲𝗿𝗳𝗲𝗿𝗲𝗻𝗰𝗲:
⁃ Release of intracellular analytes into serum/plasma
⁃ Spectrophotometric interference by free hemoglobin
⁃ Hemoglobin-mediated chemical interference
⁃ Dilution of plasma/serum components

Elevated bilirubin levels producing dark to bright yellow serum or plasma.

⁃ Hemolytic disorders
⁃ Liver disease
⁃ Biliary obstruction

⁃ Bilirubin reacts with assay reagents, altering analyte measurements
