June is Dysphagia Awareness Month! To kick off, we’re sharing the story of Glenn and Miriam from New South Wales.
 
Glenn suffered a brainstem stroke in 2024, and developed dysphagia as a result. After trying other swallowing rehabilitation methods, he used BiSSkApp to get back to a normal diet.

What do you do when a client with chronic dysphagia appears to have plateaued?

We’ve just published a new clinical case study exploring the implementation of skill-based swallowing training with biofeedback in a community TBI client living with chronic dysphagia.

After reaching a plateau in progress, but still having persistent swallowing difficulties resulting in aspiration on thin fluids and ongoing reliance on thickened fluids, the treating team trialled a different rehab approach using BiSSkApp.

The case study walks through:
• the clinical reasoning behind task selection
• programme setup and progression
• managing fatigue and cognitive factors in TBI
• the role of support workers and family
• VFSS, TWST and ToMaSS outcomes
• practical lessons learned during implementation

For clinicians working with chronic neurological dysphagia, it offers a detailed look at how a feedback-rich rehabilitation model was implemented in a real-world community setting.

One of the most common questions clinicians ask us about neurodegenerative clients is:
“How long should I keep them on an intensive program?”

There isn’t a one sized fits all answer, and while research in this area continues to grow, we can take a personalised approach and guide decisions based on progress towards their goals.

A practical framework you can trial:
• Take clear outcome measures at baseline
• During intensive periods, draw on motor learning principles. Repetition, specificity and feedback are key drivers of change.
• Re-test at defined intervals to track progress
• During maintenance periods, consider lower frequency training to support skill retention over time
• Expect to cycle between intensive and maintenance approaches, guided by patients reports of their symptoms and your objective measures.

And importantly - none of this works without adherence. If your patient can’t realistically sustain it, the “perfect” plan won’t matter.

“How many swallows should my patient be doing?”

The range is wider than most people expect. Early evidence for Parkinson's Disease suggests 100-120 per day. You can consider a spectrum from 50 to 120+ per day – less for patients that are acute or easily fatigued, and more for patients that are medically well and motivated.

But in practice, the biggest mistake isn’t under-dosing… it’s starting too high.

Beginning with something manageable gives patients a chance to succeed early. That early success is what drives adherence – and makes it much easier to build intensity over time.

From there, dosage should be adjusted based on fatigue, medical status, cognition, and what realistically fits into their day.
