Dear Patient,
To help give you safer, more connected care, West End Medical Centre will soon start using HealthOne – this is a secure shared health record that lets authorised health professionals involved in your care see the health information when and where it is needed. HealthOne has already been used in the South Island for more than 12 years.
What does this mean for you
If you receive care from another GP, a hospital, community pharmacy, or urgent care services, clinicians involved in your care may be able to view important health information such as your allergies, medications and GP summaries.
Our practice has decided to also share the consultation notes made during appointments. This information can help other clinicians understand medication changes, how symptoms have changed over time and how your medical centre is treating you.
Sometimes you may discuss something that you do not want to be made visible in a shared record. Let your clinician know and they can change the privacy setting for that consultation.
Having access to this information can help healthcare providers make more informed decisions and provide safer, more coordinated care.
Your information, your choice
Your privacy is important. You can choose whether some, all, or none of your health information is shared through HealthOne, and you can change your choice at any time.
Your information will only be shared with health professionals directly involved in your care, and only in line with your sharing choices. Any privacy or confidentiality settings you already have in place will also apply to HealthOne.
If you are happy for your health information to be shared through HealthOne, you do not need to anything.
If you want to learn more about HealthOne and your privacy options:
• Visit their website: Patient Information Systems – healthone.org.nz
• Read the HealthOne Privacy and Security information online: Privacy & Security - healthone.org.nz
• Contact the HealthOne Privacy Officer:
o Freephone: 0508 837 872
o Email: [email protected]
• Talk to your practice team

Please note that choosing not to share some, or all of your health information could affect your care, especially if you are treated by someone who does not usually see you as they may not have all the information they need to support you as well as possible.

For help and support
If you have any questions about sharing your health information, please contact us by email: [email protected] Attn: Practice Manager.

Health NZ | Public Health Advisory –
One new measles case confirmed in the Manawatū Region

An individual who lives in Palmerston North has been confirmed with measles. The case was infectious from 21 – 29 July. To date, no source has been identified for this case who has no history of recent overseas travel. This means additional cases linked to the same unknown source are possible.

Public health teams are identifying and managing close contacts linked to this case and identifying any potential locations of interest. Locations of interest will be updated here as they are identified Measles: Locations of interest in Aotearoa New Zealand.

Clinicians are asked to be alert for additional cases who could have been exposed to the same unknown source case. Although the case spent most of the incubation period in Palmerston North, they visited Wellington from 11 to 12 July, where they attended a music event at Shed 6 and visited some local bars. This was a busy weekend in Wellington, with Matariki events, several large concerts, and an All-Blacks rugby game on 11 July which could have attracted visitors from overseas. It is possible that an infectious measles case was in Wellington over this time.

Please refer to the attached public health advisory for further information.
