Managing your health, and the health of your whānau, is not always straightforward.

Sometimes what's needed isn't just a prescription or a referral, but someone to walk alongside you, help connect you with the right support, and make sure nothing gets missed along the way.

That's the thinking behind the new Local Health Care Coordination service, He Pito Oranga.

Our Care Coordinators, Alicia and Grace work closely with patients and whānau who are navigating complex health and social challenges. They join the dots between health, social services, and community support so that people don't have to figure it all out alone.

If you want to learn more about this, chat to our team on 06 357 8061 or email [email protected]

About the CGG
The CGG meets every two months, and brings together clinicians, administrators, Iwi and community members. The CGG helps ensure the quality, safety, and accessibility of Local Health services. This includes things like:

- making sure our programmes help improve patient outcomes
- identifying opportunities for improvement
- supporting staff to provide evidence-based, culturally safe care
- looking at access to services, risk management, and equity to help ensure everyone in our community receives the best possible care.

Who we’re looking for
We're keen to hear from people who are passionate about the health and wellbeing of our community.

How to apply
Send your CV and a short personal statement telling us about yourself, your experiences and why you’re interested to [email protected] by 10 July. You are welcome to email us if you have any questions.
