"Do I have to undress for all of this?"

One patient asked us that early on, quietly, the way people ask the questions they have been carrying for a while. The answer surprised her, and it made the whole decision feel a lot less daunting.

Most people arrive already told what they need. Do your Kegels. Try this device. Wait and see. We start somewhere else, by understanding the problem first, and part of that is knowing what each option actually involves before you choose anything.

Here are four things experienced patients tend to know going in.

No un******ng required. Assessments like real-time ultrasound biofeedback are done fully clothed. There is far less to dread here than most people imagine.

"I hope I am not wasting your time."

One woman said that as she sat down for her first appointment, at 75. She had spent years quietly following a set of rules that nobody ever wrote down, but almost everyone seems to follow.

Here are a few of them, and why you can stop following them now.

You must apologise for having a pelvic floor problem. These symptoms are common and worth treating, at any age. There is nothing to apologise for.

You are too old for treatment to work. Age does not decide how well you can recover. She found her problem was treatable, and she knew what to do if it ever returned.

You have to understand the exercises straight away. You do not. The technology we use lets you see exactly what your muscles are doing, so a vague instruction becomes something you can feel and repeat with confidence.

You should just manage, and not expect to get fully better. For this woman, the problem is now gone. Full resolution can be a realistic outcome.

You need to already know what is wrong before coming in. Coming in with questions is the whole point. That is what an assessment is for.

By the end of her programme, she had enough confidence to sign up for beginner Pilates, something she never thought she would try.

I spent two years convinced I was the only person dealing with this. I wasn't even close.

Looking back, it wasn't the symptom that kept me quiet. It was three quiet beliefs I never thought to question.

The first was that I was alone in it. The most isolating part was the silence around it. I had no idea that over half of women experience some degree of pelvic floor dysfunction in their lifetime. Knowing that didn't fix anything on its own, but it made it a lot easier to finally pick up the phone.

The second was that nothing could be done without surgery. I had mentally filed this under "problems that need an operation." Nobody had ever told me that pelvic floor physiotherapy is often the first-line recommendation before surgery is even considered. That one piece of information changed my whole decision.

The third was that physio couldn't help with something like this. I genuinely didn't know a physiotherapist could assess and treat it. I thought it was a GP or a surgeon, full stop. When I found out there were non-invasive options, and that I could discuss everything confidentially, the barrier I had built in my head just disappeared.

The condition was always treatable. The only thing that changed was the day I stopped assuming it wasn't.

Erectile dysfunction is not always about weak muscles.

Here is a pattern we see in clinic. A man notices erectile difficulties, assumes his pelvic floor is weak, and spends months squeezing harder to fix it. The symptoms do not shift. Sometimes they get worse.

The assumption makes sense. In most conversations about muscles, weakness is the problem and strengthening is the answer. The pelvic floor does not always follow that rule.

When these muscles carry too much tone and never fully release, they can restrict the blood flow needed for an er****on. Strengthening a pelvic floor that is already too tight pushes it further in the wrong direction.

So the first clinical question is rarely "how strong are these muscles?" It is "what is the actual tone and coordination pattern here?" Both a weak pattern and an over-tense one can contribute to ED, and they need very different care.

A proper assessment can show which one is driving your symptoms before any exercise plan begins. If you have been working at this without progress, it is worth checking what your pelvic floor is really doing.

Most people guess that around 10 to 15 percent of women develop pelvic organ prolapse in their lifetime.

The real number is over 50 percent.

That changes the whole picture. Prolapse is not a rare surgical concern. It is common, manageable, and worth talking about early rather than quietly carrying alone.

Prolapse happens when the tissues that support the pelvic organs weaken or stretch, allowing the bladder, uterus, or bowel to slip from their normal position. Many women first notice it as pressure, a dragging feeling, or a sense that something is not sitting quite right. The likelihood increases after childbirth and with age.

Surgery is rarely the only path forward. A proper pelvic floor assessment can show what your body is actually doing and where there is room to help.

Half of your peers. That is the reality most men never hear about erectile dysfunction.

Most men who come to us assumed ED was something that happened to a small, unlucky minority. The real number tells a different story. Erectile dysfunction is a common concern for over 50% of men aged 40 to 70, and more again experience it in a passing, transient way.

That number matters. So many men carry this quietly, convinced they are the exception. When you know that half of your peers are affected, it stops being a private failing and becomes a shared experience.

Erectile dysfunction is the frequent or consistent difficulty getting or keeping an er****on firm enough for s*x. The occasional off night is rarely a cause for concern. When it becomes an ongoing pattern, it can affect confidence, add stress, and quietly strain relationships.

Here is the part worth holding onto. Common does not mean you have to accept it. "Just squeeze harder and it will fix itself." It is one of the most common assumptions we hear about the pelvic floor, and it is one of the biggest myths.

The idea is that every pelvic floor problem comes down to weakness, and that more effort is the answer. For a lot of women, more effort is not what the muscles need.

Here is the part that is hard to see from the outside. As many as 25% of women bear down on their pelvic floor when they are trying to lift it. They are pushing down when they think they are drawing up.

On a real-time ultrasound screen, we can watch it happen. The bladder neck descends instead of lifting, a small but clear movement in the wrong direction. No amount of extra squeezing corrects that, because the effort is going the wrong way.

What matters is knowing what the muscles are actually doing, so the effort you put in lands where it should.

A proper assessment can show you exactly that. If "just do more" has never quite worked for you, there may be a good reason, and it is worth looking at together. ๐Ÿ’™

"I thought this kind of pain was just my life now."

We hear versions of that sentence often. Chronic pelvic pain has a way of settling in so quietly that you stop calling it a problem and start calling it normal. Pain in the lower abdomen and back that has been there so long it fades into the background of an ordinary day.

One patient shared three things she learned after she finally stopped assuming nothing could help.

First, it had a name. Finding out it was a recognised, treatable condition, rather than stress or bad posture, was genuinely surprising to her after so long.

Second, treatment did not have to mean surgery. She had put off looking into it because she assumed the only options were invasive. The care she received was non-surgical, and gentler than she had imagined.

Third, talking about it was the hardest part. Walking into a clinic to discuss pain in that part of your body can feel exposing. But the environment was matter-of-fact and without judgment. That first conversation turned out to be easier than the years she spent not having it.

The hardest part is usually the decision to go. What comes after tends to be simpler than you feared.

Stigma is the unearned shame that makes a treatable condition feel like a secret worth keeping.

It is often the reason people wait years before walking through a clinic door.

Many put off asking for help because pelvic health issues feel too private, too embarrassing, or too tied up in the idea that they should simply cope. Embarrassment and stigma around these conditions lead directly to delayed treatment, and that delay quietly wears away at quality of life.

Part of it is not knowing there is another option. Pelvic floor therapy is still not widely understood as a path forward, so symptoms often get managed alone rather than assessed and treated.

Here is what we want you to know. Leaking, urgency, pressure, pain, and s*xual difficulties are common. They are treatable. And they are nothing to be ashamed of.

You do not need the perfect words to begin. A simple "I have been noticing this, can you help?" is enough to start.

A good pelvic health clinic does not guess what your muscles are doing.

When it comes to the deep muscle layers, the deep abdominals, the back, and the pelvic floor, telling someone to "engage your core" or "lift your pelvic floor" only goes so far. These muscles sit deep and quiet. From the outside, it is very hard to know whether the right one is switching on, or whether something nearby is doing the work instead.

That is where real-time ultrasound imaging comes in. It lets us see the deep muscle layers moving on screen, in the moment, while you try the exercise. You watch alongside us.

Without it, we would be left hoping that your deeper muscle layers were being recruited correctly. With it, we can check together, adjust the cue, and know that the effort is landing where it needs to.
