This is a very familiar pathway to me.
Sadly in NZ getting help can be so hard.

If your child changes overnight go to your Dr immediately.
Get the situation documented.
Take videos.
Go to your hospital.
Show the drs the PANS PANDAS info. https://www.pandasppn.org/flowchart/
Do not be quiet.
Show them the info.
Tell them your child has had a massive change.

THANKS FOR ALL YOUR HELP.

ULTIMATELY IT FALLS ON THE SHOULDERS OF THE FAMILY TO PURSUE ALL THE AGENCIES WHO ARE SUPPOSED TO BE HELPING THEM.
MANY AGENCIES.

VERY CONFUSING FOR MANY PEOPLE, ESP ESL AND FAIRLY NEW TO THE NZ SYSTEM. THE ONE AGENCY THAT IS INVOLVED SEEMS TO HAVE LITTLE URGENCY FOR THIS WEE GUY.

Sadly I have to reach out again to try to help a client with a service. This little guy had one visit from a funded SLT, no report, no email and no follow up.

He urgently needs and SLT.
Parents cant not afford private.
He is in daycare (not a proactive one).
He is 5, still at day care until 6 yrs
Mild CP, ASD level 2, Global developmental delay
NG fed from birth to 16 mths
Has some speech, he can use short sentences.
ESL. First word aged 2.
Can not chew properly, mainly eats purees.

Can any one please give me some hints and tips on how to get him some SLT help?

Very young child.

Now has failure to thrive, anxiety, reflux, sleep issues and more......

18 antibiotics

11 nasal swabs

1 was positive

So nearly all the antibiotics were given, for VIRAL issues..
So likely fully unecessary.

Yes antibiotics can save lives
But over use and misuse can be so damaging too.

This case is not a current client, but I have a current client SO similar.. There are many children like this out there.

Also can use xylitol nose spray straight before and after kindy/school.

This tip is from Andy!

"Anti-viral support and antiseptics:

Betadine gargle.

Swab nose with a cotton bud dipped in the betadine gargle then gargle with the remainder. Do this twice daily more if surrounded by illness. This is removing the viral nest before you even get symptoms. Dr’s working in the Covid wards do this religiously."

"A clinical diagnosis requires more than the presence of inattentive or impulsive tendencies. Symptoms must be persistent, developmentally inappropriate, present across settings, and associated with meaningful impairment. Patients who do not meet full diagnostic criteria can still benefit from support addressing attention, organization, sleep, stress, mood symptoms, or other contributors to impairment."

"Not Every Attention Problem Is ADHD
The clinician must determine whether another condition better explains the attentional symptoms or whether ADHD and a co-occurring disorder are present.

Anxious thoughts and stressful ruminations often distract patients, making it harder to sustain focus on a task and exclude unrelated stimuli and information. Depression may contribute to low motivation and poor task initiation. Symptoms of depression and anxiety can be incorrectly attributed to ADHD. Conversely, ADHD may initially be diagnosed as anxiety or depression because of the overlap in symptoms and treated as such.

Substance use may cause low arousal and poor motivation that resemble ADHD. Attention can also be worsened by sleep deprivation, sleep disorders, medication effects, or other medical causes. The differential diagnosis can become particularly challenging in older adults, in whom new or worsening attention problems may also raise concerns about cognitive decline. "

Inattention doesn't necessarily mean ADHD. Dr Zebib Abraham explains the complexities of evaluating adults and what clinicians should consider before making a diagnosis.

What is a well child visit supposed to cover?
The issue is partially timing? I don't know how one of these topics can be covered fully in 15 mins, let alone all of them.

So many Drs are doing their best!!! Within a system that makes it harder to promote health due to time restraints.

A dedicated Well Child provider or nurse can often spend longer.

Who has had a really good, positive experience and felt informed and supported? We want to hear the good stuff!
