βœ…οΈ Mother and daughter M.B. 62/F and K.B. 32/F from Tarlac underwent surgery on the same day: total thyroidectomy via transoral endoscopic thyroidectomy vestibular approach (TOETVA).

βœ…οΈ Their procedures were free from complications and they were advised regular post-operative follow-up.

βœ…οΈ Thank you for trusting Maliwat ENT Medical Clinic!

βœ…οΈ A.C. 65/F is a breast cancer survivor who underwent multiple operations for her condition (breast and lung surgeries). Recently, she developed multiple enlarged neck lymph nodes which were deemed suspicious for cancer recurrence, hence was referred to our clinic.

βœ…οΈ Removal of cervical lymph nodes were done and sent for biopsy, with results revealing tuberculous lymphadenitis. Patient is currently on oral medications and has returned to her normal routine.

βœ…οΈ Thank you Mrs. A.C. and family for trusting Maliwat ENT Medical Clinic!

Are swollen neck lymph nodes outright dangerous?

πŸ’‘ Swollen neck lymph nodes are usually a temporary response to a recent head or neck infection, which are typically soft and painful, and regress in size within 2 to 4 weeks.

πŸ’‘ Opposite symptoms e.g. hard, non-painful, and persist for more than 2 to 4 weeks, could be a warning sign for a possible serious condition and warrant immediate medical attention.

(Google AI, Mayo Clinic)

πŸ’‘ Our clinic highly recommends an evaluation by a medical professional for any appearance of head and neck masses.

What is tracheostomy?
πŸ’‘A tracheostomy is a surgically created opening through the front of the neck and directly into the windpipe (trachea). A curved tube (called a tracheostomy tube) is inserted into an opening in the trachea to keep the airway open, allowing air to reach the lungs by bypassing the nose and mouth. (Cleveland Clinic)

Is tracheostomy permanent?
πŸ’‘A tracheostomy is temporary for the majority of patients, although it may become permanent if the patient has either a chronic condition, an irreversible airway damage, or a disease that requires parts of the airway to be removed.

βœ…οΈ K.P. 33/F came in to the clinic inquiring if we can perform a tonsillectomy procedure for recurrent tonsillitis. Upon assessment, diagnosis was indeed a case of recurrent tonsillitis, hence patient was subjected to a tonsillectomy procedure.

βœ…οΈ Regular follow-ups were done until patient was post-surgically cleared for regular activities and work.

βœ…οΈ Thank you for trusting us with your throat care!

What are some complications of recurrent tonsillitis?

πŸ’‘ Recurrent tonsillitis can trigger abnormal autoimmune responses leading to complications such as:

1. Rheumatic fever: an inflammatory disease that can permanently damage the heart (rheumatic heart disease), joints, and brain.

2. Poststreptococcal glomerulonephritis: an inflammatory condition of the kidneys that occurs when immune complexes from a throat infection get deposited in the kidneys, potentially leading to acute kidney issues which may lead to kidney failure requiring dialysis.

βœ…οΈ V.H. 43/M was admitted due to severe neck pain and was referred to the ENT specialist for evaluation. Assessment was a deep neck infection from a submandibular gland abcess. Surgical drainage was done which afforded immediate relief. Patient was discharged after a few days and followed up at the clinic free of complaints.

What is deep neck infection?
πŸ’‘ A deep neck infection is a potentially life-threatening bacterial infection and abscess in the deep fascial spaces of the neck. It typically originates from dental issues, tonsillitis, or salivary gland infections. If you suspect this condition, you must seek emergency medical care, as untreated infections can rapidly cause airway obstruction or spread to the chest which can lead to death. (NIH, PubMed)

βœ…οΈ D.G. 26/M came to our clinic due to complaints of recurrent sharp facial pain and frequent headaches. Several clinic consults were done prior to coming to the clinic, all of which afforded transient relief.

βœ…οΈ Complete ENT examination revealed a deviated nasal septum, hence, patient was subjected to an endoscopic septoplasty procedure.

βœ…οΈ Patient followed up 1 week after his procedure claiming resolution of facial pain and headaches.

βœ…οΈ Thank you for trusting Maliwat ENT Medical Clinic!

What is rhinologic headache?
πŸ’‘ A rhinologic or rhinogenic headache is pain originating from inflammation, infection, or structural abnormalities within the nasal cavity. Anatomical variations or mucosal contact-points such as deviated nasal septum or a bone spur are some of the common causes. Surgical correction of these abnormalities often leads to the resolution of patient's symptoms. (PubMed, NIH, Healthline)

βœ…οΈ Dr. S.C. 38/F is a case of advanced thyroid carcinoma who underwent subsequent surgeries: first was an aborted endoscopic thyroid surgery (incision biopsy performed) and second was a total thyroidectomy with bilateral recurrent laryngeal nerve sacrifice, bilateral modified radical neck dissection, and prophylactic tracheostomy.

βœ…οΈ After months of hard-fought battle due to expected and unexpected complications, patient successfully underwent both tracheostomy decannulation and adjuvant radioactive iodine (RAI) treatment. Patient is currently under disease-monitoring and active surveillance.

βœ…οΈ She is one of our prized patients: a very responsible and compliant patient surrounded with a good support system.

βœ…οΈ We are grateful for her trust. Thank you for entrusting us with your health Dr. Shee Mc!

What is radioactive iodine?
πŸ’‘ Radioactive iodine (RAI) is used in the diagnosis and treatment of thyroid conditions. In large doses, it targets and destroys remaining thyroid cancer tissue after thyroidectomy, while leaving the rest of the body mostly unaffected.

βœ…οΈ Ms. N.C. 35/F from Tacloban was referred to our clinic by an endocrinologist for total thyroidectomy due to Graves' disease.

βœ…οΈ Patient was discharged early, went for follow-up 5 days after surgery, and went home to her province after a few weeks.

What is Graves' disease?
πŸ’‘Graves' disease is an autoimmune disorder that causes the immune system to attack the thyroid gland. This attack forces the thyroid to produce excess hormones, leading to hyperthyroidism. It speeds up the body's metabolism and commonly causes anxiety, weight loss, and an enlarged thyroid.

βœ…οΈ Ms. J.C. 20/F was referred to our clinic by a specialist for the removal of enlarged tonsils which was presumed to cause obstructive sleep apnea.

βœ…οΈ Review of workups were done and patient was subjected to bilateral palatine tonsillectomy and adenoidectomy.

βœ…οΈ Patient claims improvement of sleep-related symptoms and claims satisfaction with her surgical care.

Does tonsillectomy effectively treat Obstructive Sleep Apnea (OSA)?
πŸ’‘ In children, tonsillectomy, often combined with adenoidectomy, is the gold standard and most common first-line treatment for pediatric OSA, because enlarged tonsils and adenoids are the leading cause of airway obstruction in this age group. (nih.gov)
πŸ’‘ In adults however, it is an effective treatment primarily for those who have noticeably enlarged tonsils but no severe obesity.
πŸ’‘ While not always a complete cure, it can significantly reduce the severity of OSA, decrease snoring, and improve daytime sleepiness.
πŸ’‘ It is important to note that adult OSA often involves multiple factors (e.g. anatomy of the jaw, weight, tongue size), meaning some adults may still require other forms of treatment like continuous positive airway pressure (CPAP) therapy or oral appliances.
