When assessing someone with a musculoskeletal condition, are we paying attention to more than just pain?

Emerging evidence suggests we should.

This review highlights that clinicians should consider:
• Looking beyond pain-related fear to emotions such as despair and distress.
• Understanding how patients respond cognitively, including acceptance.
• Recognising whether they rely on active or passive coping behaviours.

These factors can influence how people engage with rehabilitation and ultimately shape recovery.

That’s why this month’s The Better Clinician Project masterclass is dedicated to The Psychology of Injury.

Throughout August, we’ll explore how to recognise these psychological factors, communicate more effectively, and integrate psychologically informed care into everyday practice- without needing to become a psychologist.

Better rehabilitation isn’t just about prescribing the right exercises. It’s also about understanding the person in front of you.

If pain is not an accurate measure of tissue damage, an equally important question follows: Is pain the best measure of recovery?
Modern rehabilitation suggests the answer is no. Although pain is an important symptom that deserves careful attention, it should not be the sole outcome used to judge treatment success. Recovery is better reflected by what people are able to do—their ability to move, work, participate in meaningful activities, and regain confidence in their bodies. This shift from a pain-centred perspective to a function-centred approach represents one of the fundamental principles of contemporary physiotherapy and evidence-based rehabilitation.

That's a sentence we'd love to hear less often.

Being told you have "the spine of an 80-year-old" would make almost anyone worry.

The problem is that statements like this often create far more fear than they do clarity.

Age-related changes on an MRI are incredibly common, even in people with no pain.

Imaging can provide helpful information, but it doesn't automatically explain why you're hurting or predict what you're capable of doing.

That's why language matters.

Instead of leaving someone feeling fragile, we should help them understand what's going on, build confidence, and gradually get back to the activities they enjoy.

Your MRI is one piece of the puzzle.

It isn't your future.
