Prevention of sacral ulcer

1. Reposition regularly
Turn the patient every 2 hours in bed
If seated, shift weight every 15–30 minutes
Use the 30-degree side-lying position instead of lying flat on the back to reduce sacral pressure.

2. Use pressure-relieving surfaces
Air mattress / alternating pressure mattress
Foam mattress
Gel cushion for chair
Heel protectors if bedridden

3. Keep skin clean and dry
Clean gently after urine/stool
Dry the skin well
Apply:
barrier cream
zinc oxide
moisture protectant
Moisture greatly increases skin breakdown risk.

4. Improve nutrition
Healing needs:
high protein diet
enough calories
vitamin C
zinc
hydration
Low albumin or malnutrition delays healing.

5. Reduce friction/shear
Avoid:
dragging patient on bed
elevating head >30° for long periods (causes sliding)
Use:
draw sheet
transfer sheet
lifting devices

Management if sacral ulcer is already present
Stage 1 (red but intact skin)
Remove pressure immediately
Keep skin dry
Apply protective barrier cream
Consider sacral foam dressing
Stage 2 (blister/open shallow wound)
Clean with normal saline
Apply:
hydrocolloid
foam dressing
silicone sacral dressing
Continue pressure relief
Stage 3–4 (deep ulcer)
Needs wound care:
Debridement of dead tissue if present
Culture if infected
Appropriate dressing
Possible antibiotics if cellulitis/infection
Surgical consultation for severe ulcers
Watch for infection signs
Seek urgent treatment if there is:
foul smell
pus
fever
increasing redness
worsening pain
black tissue
