Ge***al Herpes in Pregnancy: the guideline in one practical post 👇

This topic becomes much easier when you stop trying to memorise everything and focus on timing + type of infection.

# # # The 2 key questions
1. Is this recurrent herpes or a primary infection?
2. If primary, did it happen early in pregnancy or close to delivery?

That is the whole logic of management.

✅ Recurrent ge***al herpes
Usually carries a lower risk of neonatal transmission.
Treat symptomatic recurrences with standard aciclovir if needed.
Consider suppressive aciclovir from 32 weeks
or earlier if there is a high risk of preterm birth.
In most cases, vaginal delivery is acceptable.

✅ Primary infection in the 1st or 2nd trimester
Treat the acute episode with standard aciclovir.
Later in pregnancy, consider suppressive aciclovir from 32 weeks.
Because the infection happened earlier, there is usually time before delivery, so vaginal birth may still be possible.

🚨 Primary infection in the 3rd trimester
This is the group that matters most.
Why? Because primary infection near delivery carries the highest risk of neonatal herpes.
Management usually includes:
- treatment of the primary episode
- continuing aciclovir until delivery
- planned caesarean section, especially if infection occurs within 6 weeks of delivery

👶 After birth, the baby must not be forgotten
Always risk assess the neonate and follow local neonatal guidance for:
- investigations
- ongoing monitoring
- treatment when indicated

# # # Clinical pearl
The closer a primary maternal infection is to delivery, the greater the neonatal risk.

So if you remember only one sentence from the guideline, let it be this:

Recurrent = lower risk.
Primary near delivery = highest risk.
