Alam naming mahalaga ang tuloy-tuloy na therapy para sa pag-unlad ng inyong mga anak. Kaya naman, patuloy kaming naghahanap ng mga paraan upang matulungan ang ating mga pamilya na maipagpatuloy ang therapy na kailangan ng kanilang mga anak. ๐Ÿฅนโค๏ธ

Bilang isang DSWD Service Partner, tumatanggap na ang Little Beacon Pediatric Therapy Center ng Guarantee Letters para sa mga kwalipikadong benepisyaryo!

Narito ang gabay kung paano mag-apply. ๐Ÿ‘‡

๐Ÿ“Œ STEP 1: KUMPLETUHIN ANG MGA KINAKAILANGANG DOKUMENTO

Ihanda ang mga sumusunod na requirements para sa inyong aplikasyon:

โœ… 1. Latest Medical Certificate
Ang medical certificate ay dapat ibinigay ng doctor sa huling 3 buwan. Original or Certified True Copy dapat ang isusubmit sa SWAD. Maaari ding dalahin ang pinakahuling assessment ng inyong Developmental Pediatrician kung ito ay nai-issue sa huling 3 buwan.

โœ… 2. Treatment Protocol
Nakapaloob dapat dito ang request ng kinakailangang therapy service ng bata (OT, ST, PT, Reading Intervention, o SPED Tutorial). Makikita din ito sa pinakahuling Developmental Assessment ng bata.

โœ… 3. Acceptance Letter mula sa Therapy Center
Maaaring hingin kay Ms. Shiela o kay Ms. Des sa Little Beacon.

โœ… 4. Quotation
Maaari ring hingin kay Ms. Shiela o kay Ms. Des sa Little Beacon. Kumuha muna nito bago kumuha ng Social Case Study sa munisipyo.

โœ… 5. Valid ID ng Patient
Siguraduhing may PWD ID ang inyong anak. I-photocopy ang harap at likod.

โœ… 6. Valid ID ng Guardian (photocopy ng harap at likod)
Maghanda ng valid ID ng magulang o legal guardian. Kung hindi immediate family member ang magaasikaso ng GL, kailangang magdala ng Certificate of Guardianship.

โœ… 7. Social Case Study Addressed to DSWD
Maaaring kumuha nito mula sa Social Worker ng inyong munisipyo. Unahing kumuha ng quotation dahil kakailanganin ito pag kumuha ng case study.

๐Ÿ“Œ MAHALAGANG PAALALA: Tanging ang Acceptance Letter at Quotation lamang ang mga dokumentong manggagaling sa Little Beacon. Ang iba pang requirements ay kailangang kunin o ihanda mula sa mga kaukulang tanggapan.

๐Ÿ“Œ STEP 2: PUMUNTA SA SWAD BATANGAS

๐Ÿ“ Lokasyon: SWAD Batangas, sa loob ng Capitolio, Batangas City.

Bagama't maaari ninyong ihanda ang mga requirements bago pumunta, inirerekomenda naming bumisita muna sa SWAD Batangas upang makumpirma ang kumpletong listahan ng mga kinakailangang dokumento at malaman kung may karagdagang requirements para sa inyong aplikasyon.

Sa ganitong paraan, maiiwasan ang hindi kinakailangang pabalik-balik sa pagproseso ng inyong Guarantee Letter.

Bukas ang SWAD Lunes hanggang Biyernes mula 8am hanggangb5pm. Mas mabuting pumunta tayo ng maaga dahil mjnsan ay may limit depende sa dami ng taong dumadating.

๐Ÿ“Œ STEP 3: INTERVIEW, ASSESSMENT, AT APPROVAL

Sumailalim sa interview at assessment ng DSWD upang matukoy ang inyong kwalipikasyon para sa tulong pinansyal.

Ang pag-apruba ng aplikasyon at halaga ng assistance ay nakabatay sa assessment at umiiral na mga patakaran ng DSWD.

๐Ÿ“Œ STEP 4: IBIGAY ANG GUARANTEE LETTER SA LITTLE BEACON

Kapag naaprubahan na ang inyong aplikasyon at naibigay na ang Guarantee Letter, dalhin ito sa Little Beacon upang maproseso ang paggamit nito para sa mga aprubadong therapy services ng inyong anak.

Para sa mga dokumentong manggagaling sa aming center, maaari kayong lumapit kina Ms. Shiela o Ms. Des. ๐Ÿ’™

We understand that for many families, continuing therapy is not always easy, especially when financial challenges arise. We never want finances to be the reason a child misses out on opportunities to learn, grow, communicate, and become more independent. โค๏ธ

That is why, aside from actively seeking sponsorships and scholarships from generous individuals, organizations, and NGOs, we made sure to process our DSWD Service Provider accreditation, too.

Our SLPs, teachers Aileen, Naomi, Toni, and Vincent, volunteered with the Philippine Association of Speech Pathologists in today's CAREvan by National Council on Disability Affairs. This event is happening simultaneously all over Luzon, Visayas, and Mindanao. Over 30 kids will be seen from the province of Batangas for assessment by SLPs, OTs, PTs, psychologists, and doctors in this event.
