■3. GATHER THE FOLLOWING REQUIRED DOCUMENTS NOW, to prepare for filing of your DOH application in APRIL 2025:
▪︎ a] Original copy of your PSA BIRTH CERTIFICATE (and Marriage Certificate, if married female).
▪︎ b] CERTIFIED TRUE COPY of your high school (or college) diploma OR Transcript of Records (Form 137).
▪︎ c] NBI CLEARANCE.
▪︎ d] BARANGAY CERTIFICATE of Good Moral Character.
▪︎ e] MEDICAL CERTIFICATE from a government physician, stating that you are "fit to take DOH examination".
▪︎ f] PASSPORT PHOTOS with name (4 pcs).

■4. Lectures for the Written Examination will be delivered by instructor , Jay Gungob, Face-to-face . Hands-on training for the Oral-Practical Examination will be conducted face-to-face with an ONSITE TUTOR.

■5. If you are a college graduate, you may avail of a Guided Self-Study Program.

If you pass both the written and oral-practical examinations, you must take the professional oath for massage therapists. Only after that will you be granted your CERTIFICATE OF REGISTRATION -- ie, the license to practice massage therapy in the Philippines.
