📢 HEA CLAIMING ANNOUNCEMENT
Inactive personnel eligible for the Health Emergency Allowance (HEA) for August 2022 and December 2022 may claim their allowance on:
🗓 April 29, 2026
⏰ 9:00 AM – 3:00 PM
📍 4th Floor Auditorium, J.R. Borja General Hospital
Please ensure all required documents are prepared for a smooth claiming process:
1. TO THOSE RESIGNED/RETIRED PERSONNEL WHO ARE IN THE CITY, please secure a photocopy of any valid ID with three (3) signature specimens for each month of your HEA.
2. TO THOSE RESIGNED/RETIRED PERSONNEL WITH AUTHORIZED REPRESENTATIVE (SPOUSE, PARENTS, AND CHILDREN), please secure a photocopy of any valid ID of the employee and authorized representative with three (3) signature specimens for each month of your HEA and Authorization Letter.
3. TO THOSE RESIGNED/RETIRED PERSONNEL WITH AUTHORIZED REPRESENTATIVE (RELATIVES, SIGNIFICANT OTHER, COLLEAGUES, AND FRIENDS), please secure a photocopy of any valid ID of the employee and SPECIAL POWER OF ATTORNEY (SPA).
4. FOR DECEASED PERSONNEL, spouse, parents, siblings, or next of kin must secure a copy of the death certificate and a photocopy of any valid ID for each month of the HEA.
⚠️ Please be informed that those who will not be able to claim their Health Emergency Allowance (HEA) on the abovementioned date must proceed to the City Hall Finance Office to claim their allowance UNTIL MAY 5, 2026 ONLY.
