PROSTATE EXAM IN A NUTSHELL 👆🏼

Sa digital re**al examination or DRE, ito ang mga chine-check sa prostate:

☑️ Size – normal ba o enlarged?
☑️ Symmetry – pantay ba ang dalawang lobes?
⭕ Surface – smooth ba o irregular?
⚠️ Consistency – soft, firm o hard?
➖ Median groove – present, shallow o absent?
🔎 Nodules – may matigas o suspicious na bukol ba?
😣 Tenderness – masakit ba kapag hinawakan?
📍 Extension – may involvement ba ng surrounding tissues o seminal vesicles?

Possible findings:

✅ Normal prostate: Normal-sized, smooth, symmetrical at walang bukol.
🟢 Possible BPH: Enlarged pero smooth, soft-to-firm at usually symmetrical.
⚠️ Suspicious for cancer: Hard nodule, asymmetry o irregular surface—even if normal ang size.
🚨 Advanced suspicious finding: Markedly enlarged, hard at irregular prostate, with possible seminal-vesicle involvement.

Ang DRE ay nagbibigay ng important clues, pero hindi ito sapat para makumpirma ang prostate cancer. Dapat itong i-correlate sa symptoms, PSA, imaging at biopsy kung kinakailangan.

A few seconds of examination may reveal an important warning sign.

This band of prostatic tissue can be seen during repeat endoscopic procedures in patients who previously underwent TURP, in this case >10 years ago.

Prostate tissue regrowth after TURP can cause urinary symptoms again. With HoLEP, mas kumpletong natatanggal ang nakabara, parang pagbabalat ng orange from its capsule.

⏱️ GOLDEN PERIOD: 4–6 HOURS!

Testicular torsion happens when the spermatic cord twists, cutting off the blood supply to the testicle.

Warning signs include sudden severe testicular pain, pamamaga, nausea or vomiting, at isang testicle na mas mataas ang puwesto.

Don’t wait for the pain to go away. Kapag biglang sumakit nang matindi ang bayag o may napansing kakaiba, pumunta agad sa emergency room. Every minute matters!
