Your Dentist May Spot This Early Disease Before Symptoms Appear

Chronic kidney disease is often called a "silent disease" because it can develop for years without noticeable symptoms. By the time symptoms appear, kidney function may already be significantly reduced.

A 2026 population-based study involving more than 6,100 adults found that severe periodontitis was associated with reduced kidney function and albuminuria, an early marker of chronic kidney disease.

Researchers observed that severe gum disease became more common as kidney function declined, and the association remained even after accounting for factors such as age, diabetes, smoking, and s*x.

Scientists believe chronic inflammation, immune responses, and oral bacteria entering the bloodstream may help explain this relationship.

These findings highlight how the mouth can provide valuable clues about overall health, reinforcing the growing connection between oral health and systemic disease.

Brushing Your Teeth May Help Protect More Than Your Smile. It May Also Help Protect Your Heart.

Most people brush their teeth to prevent cavities and gum disease.

But growing research suggests that this simple daily habit may also be associated with better cardiovascular health.

A study published in the International Journal of Cardiology—found that adults who brushed their teeth at least twice a day for two minutes had a significantly lower risk of major cardiovascular events. In contrast, people who brushed less than twice daily and for less than two minutes had about a threefold higher risk of cardiovascular death, heart attack, heart failure, or stroke, even after adjusting for multiple cardiovascular risk factors.

Scientists believe one possible explanation is inflammation. Poor oral hygiene allows dental plaque to accumulate, increasing the risk of gum disease. Chronic gum inflammation may enable oral bacteria and inflammatory molecules to enter the bloodstream, where they could contribute to blood vessel dysfunction and cardiovascular disease. However, it is important to remember that this study shows an association, not proof that brushing itself prevents heart disease. Other healthy lifestyle factors may also play a role.

The good news is that brushing twice a day with fluoride toothpaste for at least two minutes is a simple, inexpensive habit already recommended for maintaining healthy teeth and gums. It may also support overall health as part of a heart-healthy lifestyle.

🦷 6 most common reasons for wisdom tooth removal.

Wisdom teeth are the last permanent teeth to erupt, usually between the ages of 17 and 25. While some grow normally and remain healthy for life, others may not have enough space to erupt properly. Contrary to popular belief, not every wisdom tooth needs to be removed. Healthy, fully erupted wisdom teeth that are properly positioned and easy to clean may not require extraction. The decision should always be based on a clinical examination and dental X-rays.

A dentist or oral surgeon may recommend wisdom tooth removal if one or more of the following problems are present:

✅ 1. Pain – Persistent pain or discomfort at the back of the mouth.

✅ 2. Impacted Wisdom Tooth – A tooth trapped beneath the gums or jawbone that may damage surrounding tissues.

✅ 3. Repeated Gum Infection – Partially erupted wisdom teeth can trap bacteria and food, leading to recurring infections (pericoronitis).

✅ 4. Tooth Decay – Their difficult-to-clean position increases the risk of cavities in both the wisdom tooth and the neighboring second molar.

✅ 5. Damage to Nearby Teeth – An impacted wisdom tooth may press against adjacent teeth, causing decay, root damage, or bone loss.

✅ 6. Cysts or Tumors – Although uncommon, impacted wisdom teeth can sometimes be associated with cysts or, rarely, tumors that damage the surrounding jawbone.

Every wisdom tooth is different. Some never cause problems, while others can silently affect neighboring teeth and oral health. Regular dental check-ups and panoramic X-rays help detect potential issues early, allowing treatment decisions to be made before serious complications develop. The goal is not to remove every wisdom tooth—but to preserve long-term oral health through evidence-based care.

🩸 Post-Extraction Hemorrhage: The INBDE Favourite Emergency! 🦷

A patient calls your clinic 3 hours after an extraction and says, “Doctor, the bleeding won't stop!”

What do you do?

Post-extraction hemorrhage is one of the most commonly tested dental emergencies because it requires rapid diagnosis, identification of the cause, and appropriate management.

🔑 High-Yield INBDE Points

✅ Primary hemorrhage
• Occurs during extraction
• Usually due to vessel injury or soft tissue trauma

✅ Reactionary hemorrhage
• Occurs within a few hours
• Caused by loss of vasoconstrictor effect, increased blood pressure, or clot displacement

✅ Secondary hemorrhage
• Occurs days later
• Most commonly associated with infection

✅ First-line treatment
• Direct pressure with sterile gauze for 20–30 minutes

✅ Local hemostatic measures
• Gelatin sponge
• Oxidized cellulose
• Collagen plug
• Tranexamic acid

✅ Figure-of-eight suture
• Stabilizes the clot
• Helps control socket bleeding

✅ Patients on anticoagulants
• Do not stop medications without appropriate medical consultation
• Local hemostatic measures are usually effective

🎯 Exam Pearl
Remember:

Primary = Immediate
Reactionary = Hours Later
Secondary = Infection Days Later

📚 MCQ 1

A 45-year-old man returns 4 hours after extraction of a mandibular molar. The extraction was uneventful. The socket is oozing blood, and the patient reports that bleeding began after the numbness wore off.

What is the MOST likely type of hemorrhage?

A. Primary hemorrhage
B. Hemophilic hemorrhage
C. Secondary hemorrhage
D. Reactionary hemorrhage

━━━━━━━

📚 MCQ 2

A 32-year-old patient presents 9 days after extraction with pain, foul taste, swelling, and renewed bleeding from the socket.

What is the MOST likely cause of the hemorrhage?

A. Vasoconstrictor wear-off
B. Hypertension
C. Infection with clot breakdown
D. Platelet dysfunction

A 25-year study of 6,000+ adults found that regular flossing was linked to a lower risk of ischemic stroke and atrial fibrillation (AFib).

Researchers believe inflammation from poor oral health may play an important role in the mouth–body connection.

Compared to non-flossers, regular flossers showed:
• 22% lower risk of ischemic stroke
• 44% lower risk of cardioembolic stroke
• 12% lower risk of AFib

Another reminder that oral health is deeply connected to systemic health.

The Indian Health Service (IHS) has announced it will stop using dental amalgam in its facilities by 2027. This move aligns with global efforts like the Minamata Convention on Mercury, which aims to reduce mercury use in healthcare and protect the environment.

Meanwhile, the American Dental Association (ADA) maintains that amalgam remains a safe, durable, and cost-effective option when used appropriately. It has been trusted for over 150 years, especially for restoring back teeth that endure strong chewing forces.

Experts generally agree that existing fillings do not need removal unless there is damage or decay. While modern dentistry is increasingly shifting toward tooth-colored composites, treatment decisions still depend on each patient’s specific needs and long-term oral health goals.

Braces can fix your teeth alignment—but without proper care, they can increase the risk of cavities and gum disease.

Orthodontic brackets create plaque traps around every tooth. If cleaning isn’t precise, bacteria build up along the gumline and around brackets, leading to early white spot lesions (demineralization), cavities, and inflamed, bleeding gums. What may look like “just stains” during treatment can become permanent enamel damage once the braces are removed.

That’s why oral hygiene during braces matters more than ever. Cleaning around brackets, between teeth, and along the gums isn’t optional—it’s what protects your enamel while your teeth are moving. Straight teeth are important, but keeping them healthy is what truly defines a successful result.

👉 Post-extraction care is just as important as the procedure itself.

Whether it’s a simple tooth extraction or a more complex surgical removal, the healing process requires careful attention to avoid complications like infection, excessive bleeding, or dry socket (alveolar osteitis).
