Normal Post-Extraction Procedure

🔹 Remove all loose debris from socket:
➟ Bone spicules
➟ Tooth fragments
➟ Restoration pieces
➟ Calculus

🔹 Check buccal and lingual gutters and tongue for debris

Socket compression
🔹 Compress socket with fingers after extraction
🔹 Restores normal alveolar width after buccal plate expansion
🔹 Residual ridge recontouring occurs mainly by labial/buccal cortical bone resorption

Suturing
🔹 Usually not required for a simple extraction socket
🔹 Suture if:
➟ Papillae are excised
➟ Severe gingival bleeding
➟ Gingival cuff is torn or loose

Pressure dressing
🔹 Place slightly moistened folded gauze over socket
🔹 Patient bites firmly for 30–60 minutes

Post-op instructions
🔹 Give written instructions
🔹 Most common cause of post-extraction bleeding = failure to follow instructions

Pain control
🔹 Prescribe analgesic if pain is anticipated

Persistent bleeding
🔹 Ask patient to bite on a tea bag
🔹 Tannic acid helps promote hemostasis

High-yield pearl
🔹 Simple extraction usually does not need sutures
🔹 Pressure gauze for 30–60 min + proper instructions prevents most post-op bleeding

🚨 A Tooth Infection Can Spread Beyond the Mouth and Become Life-Threatening

A tooth infection is more than a painful tooth. If left untreated, bacteria can spread beyond the tooth into nearby tissues and, in severe cases, enter the bloodstream, triggering a dangerous body-wide inflammatory response.

Most tooth infections begin when bacteria enter the pulp through deep decay, a cracked tooth, trauma, or a leaking restoration. As the bacteria multiply, they can form a dental abscess—a pocket of pus at the root of the tooth or in the surrounding gum.

Warning signs that require prompt dental evaluation include: • Severe, persistent, throbbing tooth pain • Swelling of the face, jaw, or gums • Fever • Pain while chewing or biting • Difficulty swallowing or breathing

Although uncommon, a severe untreated dental infection can spread into deeper tissues. In rare cases, bacteria may enter the bloodstream and contribute to sepsis—a life-threatening medical emergency that requires immediate treatment.

Early dental care, including drainage of the infection, root canal treatment, or tooth extraction when necessary, can stop the infection from spreading and help prevent serious complications.

Many parents use a bedtime bottle to help their baby fall asleep, but this common habit can quietly increase the risk of Early Childhood Caries (Baby Bottle Caries)—one of the most preventable dental diseases in young children.

When a baby falls asleep with milk, formula, juice, or other sweetened drinks in the mouth, natural saliva production drops during sleep. With less saliva to wash away sugars, cavity-causing bacteria have hours to produce acids that gradually dissolve the thin enamel of baby teeth. Because primary teeth are more delicate than permanent teeth, decay can progress surprisingly quickly.

Baby teeth are not "temporary" in terms of importance. They help children chew comfortably, speak clearly, maintain proper jaw development, and preserve space for permanent teeth. Untreated decay can lead to pain, infection, difficulty eating, disturbed sleep, and premature tooth loss that may affect future dental alignment.

Simple daily habits can dramatically reduce the risk. Avoid putting babies to bed with bottles containing milk, juice, or sweetened drinks—water is the safest bedtime option. Clean the gums before teeth erupt, brush as soon as the first tooth appears with an age-appropriate amount of fluoride toothpaste, transition from bottles to cups around the first year of life, and schedule the first dental visit by age one.

A few preventive steps during infancy can protect a child's smile for years to come.

🦷 The Biggest Mistake in TMJ Exercises?
⚠️ Opening your mouth as wide as possible without control.
In TMJ rehabilitation, the quality of movement is more important than the amount of movement.
🎯 Controlled Mouth Opening
✅ Goal
➡️ Open the mouth slowly and in a straight line.
➡️ Maintain smooth, symmetrical movement.
➡️ Avoid jaw deviation during opening.
⚠️ Why Is This Important?
❌ Uncontrolled opening may increase joint loading.
❌ It may overload the articular disc and surrounding muscles.
❌ Repeating poor movement patterns may delay recovery.
✅ How to Perform the Exercise
1️⃣ Sit upright with good posture.
2️⃣ Place the tip of your tongue gently on the roof of your mouth just behind the upper front teeth (if recommended by your clinician).
3️⃣ Open your mouth slowly while keeping the jaw moving in a straight line.
4️⃣ Stop if you notice pain, clicking that worsens, or jaw deviation.
5️⃣ Close the mouth slowly with the same controlled movement.
💡 Clinical Tip
🪞 Perform the exercise in front of a mirror.
➡️ This helps you detect jaw deviation early and improve movement control.
⚠️ Important Note
🔹 TMJ exercises are not the same for every patient.
🔹 The appropriate exercise depends on the diagnosis (muscular disorder, disc displacement, joint inflammation, hypermobility, etc.). Performing the wrong exercise may aggravate symptoms in some cases.
🎯 Remember:
"Open with control… not just wider."
"افتح فمك بتحكم… وليس لمجرد أن تفتحه أكثر."

Abfraction vs Abrasion: Know the Difference

Abfraction and abrasion are both non-carious cervical lesions (NCCLs), but they differ in their underlying cause and clinical presentation.

Abfraction appears as a deep, wedge-shaped cervical defect with sharp internal angles. It has traditionally been attributed to occlusal stress and tooth flexure that concentrate forces at the cervical region. However, current evidence suggests that abfraction is likely multifactorial, with occlusal loading acting alongside other factors rather than being the sole cause.

Abrasion is caused by mechanical wear from external factors such as aggressive tooth brushing, abrasive toothpaste, improper brushing technique, or repetitive habits. These lesions are typically broad, shallow, smooth, and have rounded margins.

Accurate diagnosis requires correlating the lesion's appearance with the patient's clinical history, oral hygiene practices, occlusion, parafunctional habits, dietary factors, and overall risk profile, as multiple processes may coexist in the same patient.

This is why even a small cavity should never be ignored.

A cavity may look small on the surface, but that doesn't always reflect the amount of damage hidden inside the tooth.

Unlike enamel, the dentin beneath it is softer and less mineralized, allowing tooth decay to spread more quickly once bacteria break through the outer protective layer. Because this process often occurs silently, many cavities continue to grow without causing pain or obvious symptoms.

By the time a tooth starts to hurt, the decay may have already reached the pulp—the living tissue that contains nerves and blood vessels. At that stage, treatment is often more complex and may require root canal treatment or, in severe cases, tooth extraction.

The good news is that most cavities are highly treatable when detected early. Regular dental checkups and timely treatment can preserve more of the natural tooth, reduce treatment costs, and help prevent avoidable complications.
