What important data can you achieve from the heart work up ?
12 LEAD EEG ( ELECTROCARDIOGRAM )
It records electrical activity of the heart.
It can detect rhythm and conduction abnormality or structural abnormality like hypertrophy or dilatation ( enlargement of the heart ) or ischemia ( heart's lack of oxygen) due to coronary artery obstruction.
2D ECHO DOPPLER STUDY
It is an ultrasound of the heart.
It is one of the most widely used diagnostic tests in cardiology. It can provide a wealth of helpful information, including the size and shape of the heart , pumping capacity, and the location and extent of any tissue damage.
Not only can an echocardiogram create ultrasound images of heart structures, but it can also produce accurate assessment of the blood flowing through the heart by Doppler echocardiography, using pulsed or continuous wave Doppler ultrasound. This allows assessment of both normal and abnormal blood flow through the heart. Color Doppler as well as spectral Doppler is used to visualize any abnormal communications between the left and right side of the heart, any leaking of blood through the valves (valvular regurgitation), and to estimate how well the valves open (or do not open in the case of valvular stenosis).
HOLTER MONITOR ( 24 hours) - to detect cardiac arrhythmia - abnormal heart rhythm
Cardiac arrhythmia, also known as cardiac dysrhythmia or irregular heartbeat, is a group of conditions in which the heartbeat is irregular, too fast, or too slow.
Many types of arrhythmia HAVE NO SYMPTOMS.
When symptoms are present these may include palpitations or feeling a pause between heartbeats. More seriously there may be lightheadedness, passing out, shortness of breath, or chest pain. It may predispose a person to complications such as stroke , heart failure or sudden death
AMBULATORY BLOOD PRESSURE MONITORING ( 24 hours) - monitor variation in blood pressure during day and night
The recently released National Institute of Health and Clinical Excellence (NICE) guidelines published in the United Kingdom recommend that a diagnosis of primary hypertension should be confirmed with 24-hour ambulatory blood pressure monitoring rather than by relying solely on office blood pressure measurement.
A mean (average) 24 hour BP of ≥130/80 mmHg is regarded as elevated blood pressure.9
Ambulatory blood pressure monitoring allows blood pressure to be intermittently monitored during sleep, and is useful to determine whether the patient is a dipper or non-dipper
DIPPER: blood pressure falls at night compared to daytime values.
NON-DIPPER : blood pressure declines less than 10% during the night compared with what it was during the day.
Nocturnal Hypertension : (ESH/ESC) guidelines defined bp ≥120/70 mm Hg during the night
Non-dipping blood pressure and nocturnal Hypertension is associated with a higher risk of left ventricle hypertrophy ( heart enlargement) and cardiovascular mortality/ death.
