To all Incoming First-Year Students:

Please be informed that the School Clinic will be closed on June 29–30, 2026 due to activities related to the enhancement of health services for our stakeholders.

As a result, the processing of medical requirements and clinic clearance for enrollment will be temporarily suspended during these dates.

Enrollment-related clinic services will resume on July 1, 2026.

We apologize for any inconvenience this may cause and appreciate your understanding and cooperation. Rest assured that these activities are being undertaken to further improve the quality of health services provided by the School Clinic.

CLINIC ANNOUNCEMENT FOR ENROLLMENT OF INCOMING FIRST-YEAR STUDENTS:

To all incoming first-year students who will be processing their enrollment:

A designated area and personnel outside the Clinic have been assigned to receive your enrollment forms. Please prepare the following:

1) Your filled-out Enrollment Form (Please ensure that your Enrollment Form has been evaluated and signed by your respective department before proceeding to the Clinic)
2) The original copies of your medical requirements bearing the School Nurse's signature

Upon submission, please be seated in the chairs designated for enrollees. If no seats are available, kindly wait in an area where you will not obstruct walkways or cause congestion, as the Clinic also caters to students and clients with other clinic-related transactions.

There is no need to worry while waiting for the release of your forms. Your name will be called by the assigned personnel once your documents have been reviewed, checked, and signed by the School Nurse.

Your patience and cooperation are greatly appreciated as we strive to provide efficient and orderly service to everyone.

CLINIC ANNOUNCEMENT

To all incoming first year students who were unable to submit their medical requirements on or before the deadline:

You may still process and submit your medical requirements from June 15 to June 30, 2026. However, submissions will be accepted only according to the schedule assigned by the Clinic.

Please note that beginning June 15, 2026, the Clinic's priority will be:

> Students who have already submitted their medical requirements and are scheduled for enrollment;

> Students who are scheduled for blood pressure (BP) monitoring; and

> Students who have appointments to consult with the School Physician.

As the enrollment period officially starts on June 15, students who wish to submit their medical requirements during regular Clinic days (Monday to Thursday) must wait for their assigned schedule from the Clinic. This is to ensure an orderly flow of services and to avoid overcrowding.
Please follow the instructions regarding the assigned schedule to be posted or announced outside the school clinic because only one clinic staff is assigned for the individual medical assessment.
Thank you for your patience, understanding, and cooperation.

Note: The Clinic reserves the right to prioritize students with scheduled medical appointments and enrollment-related concerns.

Please be reminded that the College of Education will have a medical orientation tomorrow, June 10, 2026 at 8:00 AM at the Student Activity Center. To those incoming first year students from other department, you can attend the said orientation.

This is to inform all incoming first-year students that the deadline for the submission of medical requirements has been changed from June 29, 2026 to June 11, 2026, as the enrollment for first-year students will now begin on June 15, 2026.

Please be advised that starting June 15, 2026, no submission of medical requirements will be entertained.

Kindly comply within the given schedule to avoid any inconvenience.

RE: SUBMISSION OF MEDICAL REQUIREMENTS

Please be guided by the following instructions regarding the submission of your medical requirements:

βœ… REQUIRED DOCUMENTS
Prepare BOTH the original copies and photocopies of the following:

1. Chest X-ray Reading - (recent result)
(Film/photo of the lungs is NOT included β€” reading/result only)
2. Complete Blood Count (CBC) Result - (recent result)
3. Drug Test Result (results from February-May are accepted)
4. Physical Examination Report / Medical Examination Report (NOT a Medical Certificate) - (recent result)
>>> For BTLED and BSHM - there will be an additional laboratory procedure of Stool Exam
>>> You need to bring any identification card and a bottled water for the laboratory procedures.
>>> You can visit the school clinic for the laboratory request form/referral
>>> Recommended diagnostic laboraties for your medical requirements: CCDI & Clinica Prime - Consolacion; Elishima Medical Laboratory - Compostela; and St. Augustine Laboratory - Carmen

πŸ“Œ IMPORTANT REMINDERS
β€’ Please photocopy all four (4) required results before submission.
β€’ File the original copies separately from the photocopies.
β€’ Come to the School Clinic as early as possible once your results are available.
β€’ The validity of the CBC result is only seven (7) days. Beyond this period, you will be required to repeat the test.
β€’ Only the SCHOOL DOCTOR is authorized to issue the Medical Certificate.
β€’ Payment of β‚±80.00 for the Medical Certificate shall be made at the Cashier’s Office.

πŸ“ SUBMISSION PROCESS
β€’ Outside the clinic, there are designated lined chairs for incoming first-year students submitting medical requirements.
β€’ Please follow the instructions of the clinic staff accordingly.
β€’ Clinic forms will be distributed during the process.
β€’ Sample accomplished forms will also be provided for your guidance in filling out the forms correctly.
β€’ Wait for your turn once seated in the designated area. Names/instructions will be announced accordingly.

πŸ“’ Attention: Incoming OJT Students (BSHM and BSTM)

You are advised to start processing your medical requirements as early as possible, even though OJT enrollment has not yet been scheduled. This is to help avoid long queues in the clinic, especially with the upcoming medical examinations of first-year students, and to prevent possible delays should there be any concerns or findings in your laboratory results.

We also encourage all class mayors to visit the clinic to receive proper instructions and secure the necessary laboratory request forms.

The School Clinic and the Office of Gender and Development proudly recognize the winners of the Poster-Making Contest held last April 16, 2026 at the HM Laboratory, in celebration of Women’s Month with the theme β€œWomen Leading Change: Smoke-Free Homes, Healthier Communities.”

Today, following the flag ceremony, a short program was conducted for the announcement and awarding of winners. This activity also supports our ongoing respiratory health awareness campaign, highlighting the vital role of women in promoting healthier, smoke-free communities.

Congratulations to all winners and participants for promoting health through creativity!
