Announcement on ROJoson F2Fmedical Consultation:
I hold Face-2-Face Medical Consultation in Manila Doctors Hospital DSMT Rm 301. Regular schedule: Tuesday: 8 to 12 nn (with prior booking). Thursday and Saturday: by appointment.
Procedure on booking for appointment: Book with my secretary, Ms. Beth Tumambing, at Telephone Number: 85224713 (Mon, Tues, Wed and Fri).
The F2Fmedical Consultation will be strictly on an appointment basis. No walk-in allowed. I will see only 10 patients on a Tuesday morning. Book at least 4 days before. First come first serve on booking.

GUIDE 7 -- TC Incidence and/or Overdiagnosis
History of the Term "Overdiagnosis" as it Applies to Thyroid Cancer

The word "overdiagnosis" refers to over-screening the population for a particular disease, which if left alone would likely not cause death anyways. It may also apply in situations when thyroid nodules are noticed when patients are given a scan for a different reason (such as a CT scan for a shoulder injury).

In 2006 two authors (Davies & Welch) co-wrote an extremely influential document entitled "Increasing Incidence of Thyroid Cancer in the United States, 1973-2002" which was published in the prestigious JAMA medical journal. This document began the trend towards explaining the rise in rates of TC as over-diagnosis, and also the tendency towards thinking that operating on small TC tumours may be over-treatment. The authors drew their conclusions based on epidemiological data.

The main points of the Davies-Welch theory of overdiagnosis are:

• Rates are rising primarily in patients with small, stage 1, PTC tumours. Many of those patients are "asymptomatic".
• If radiation exposure is the cause or risk factor for thyroid cancer, they surmise that in order for the rates to truly go up, there must be more radiation in our environment, which they argue is not the case.
• They cite autopsy studies which find at least 36% of people who die in old age of something non-thyroid related, do in fact have thyroid cancer tumours once their gland is examined.
• They claim the rise in the incidence rate is really a reflection of increased "diagnostic scrutiny" starting in the 1980s.
• They say more women are "overdiagnosed" than are men. They are more "burdened" by the diagnoses.
• They say overdiagnosis also leads to overtreatment. That is, very small tumours which may have remained "indolent" if left alone are removed in a TT surgery with all possible surgical after-effects and lifelong hormone replacement.

They conclude that the discovery of a 1cm tumour should be left alone and classified as a "normal finding".
