To celebrate Father’s Day, some of our kids created special crafts for their dads and father figures. ✨

While having fun and expressing their creativity, they also worked on important skills such as fine motor coordination, scissor use, bilateral coordination, visual-motor integration, attention, and following directions. ✂️🌱

Every cut, color, and little detail was made with love.

Happy Father’s Day to all the dads, grandfathers, and father figures who support, encourage, and cheer on our little ones every step of the way. 💙

We’re grateful for the families who have trusted us over the past month. Seeing children learn, play, and grow in their own unique ways has been one of the most meaningful parts of our journey so far. ✨

📢 THE MEDICAL CITY DEVELOPMENTAL ASSESSMENT
(Para sa INDIGENT PEDIA patients, 0–17 years old)

📌 DISCOUNTED RATES para sa mga maaaprubahan ng Social Service.

📄 IHANDA ANG MGA SUMUSUNOD:

1. Certificate of Indigency
• Mula sa barangay
• Nakapangalan sa magulang/tagapangalaga
• Nakalagay ang purpose na “Medical Assistance”

2. Certificate of Residency

3. Valid ID ng magulang/tagapangalaga

4. Isa sa mga sumusunod na identification document ng bata:
• PWD ID
• School ID
• Birth Certificate

5. Proof of Income ng magulang/tagapangalaga
(Sakop ang nakaraang 3 buwan)

Para sa Proof of Income, maaaring isumite ang mga sumusunod:
• Payslip
• Certificate of Employment (dapat nakalagay ang monthly salary)
• Contract of Employment with payslip (para sa mga tagapangalaga na nasa ibang bansa halimbawa seaman, OFW, at iba pa)
• Kung walang payslip o proof of income, maaaring magsumite ng billing statement (hal. electricity o water bill) na may parehong home address

📌 Kung kamag-anak ang tumutulong sa gastusin, kailangan din ang kanilang proof of income.

• Mahaba po ang pila para sa interview ng Social Worker.
• Isang registration lamang po bawat pasyente.

📌 Ang registration ay HINDI automatic na schedule para sa Developmental Assessment. Kailangan munang ma-interview at ma-assess ng Social Worker upang malaman kung qualified para sa serbisyong ito.
