“Feeling OK” doesn’t mean you’re OK — and that’s how readmissions happen.

A patient gets discharged, feels fine, skips the follow-up, stops the meds once symptoms disappear. Weeks later — back in the ER. Same diagnosis, same complications, sometimes worse.

Hypertension, heart failure, diabetes, liver cirrhosis — these don’t just go away after one hospital stay. Feeling better is not the same as being stable. The condition is often still there, quietly progressing, even without symptoms.
