💀👻DANGER SIGNS and SYMPTOMS SERIES💀👻

UNRESPONSIVE AND NOT BREATHING/ONLY GASPING AND NO SIGNS OF LIFE => CPR

❌RESPONSIVE
💚answer, move, moan = responsive => hindi need CPR
Ang CPR ay para po sa cardiac arrest at HINDI para sa wala lang malay or nahihilo or masama ang pakiramdam. Kung nagcollapse tapos when you shouted the name at tinap sa shoulder ay dumilat ang mata or nagpakita ng any sign na buhay siya, HINDI CPR ang kailangan niya.
😡May mga videos online na inaalis na ng patient ang kamay ng nagCCPR kasi nasasaktan siya pero tuloy pa rin sila ng CPR.

❌UNRESPONSIVE pero breathing and may signs of life => NO need CPR
ex. mga teens with psychiatric problems na nirefer na namin sa psychiatrist pero hindi naman dinadala ng magulang -> tapos magkukunwari silang unresponsive to seek attention or reaction to strong emotion -> ipapacheck up niyo pa rin sa ER para sure pero HINDI need CPR + sundin niyo ang advice na ipacheck sa psych dahil paulit ulit lang din iyang mangyayari kung hindi niyo pinapagamot ang psychiatric cause
- Example lang po iyan. WALA po akong sinasabing lahat ng ganyan ay psychiatric at HINDI ko rin sinasabing hindi emergency ang unresponsiveness na may breathing and signs of life. Hindi lang need CPR pero need pa rin dalhin sa ER para malaman ang cause.

❌NO PULSE CHECK FOR LAY RESCUERS
- lay rescuer = ordinaryong tao na hind health professional ex. magulang, g**o, lifeguard
- starting 2000, inalis na po ng American Heart Association ang pulse checking... HINDI na ito basis kung gagawin ang CPR
- kahit sa latest 2025 AHA guidelines hindi rin ito recommended "THEY SHOULD NOT CHECK FOR A PULSE"
- Napakababa ng accuracy ng pulse check by lay rescuer.
ex. nadedelay ang CPR kasi hindi sure kung tamang lugar ba ang chinecheck or akala may pulse pero pulse ng sariling daliri pala ang nararamdaman at hindi sa patient

❌IRREVERSIBLE DEATH
- kapag obvious nang hindi reversible ang death, wala nang sense gawin ang CPR
- ex. matigas na, nabubulok na, etc.

🤓Collapsed patient man or other emergencies, ang first step ay check for danger first.
🤓Nelson Pediatrics 2024 - "... provider’s first task is a quick survey of the scene itself ... provider’s and child’s safety have been ensured..."
❌HINDI una ang checking for responsiveness, pulse check, etc.
🤓ex. chemical spills, crime scenes, exposed wires, fire, natural disasters , oncoming traffic, etc

⚡️ex. nakuryente ang bata - Patayin mo muna ang kuryente. Kung hinawakan mo to check responsiveness, e di nakuryente ka rin.

🤓step 1: Timbangin ang bata in kg.

Compare the current weight with the last known weight kasi kung sobrang layo baka sira ang timbangan niyo or mali ang pagtimbang mo.

❗️For emphasis, ulitin ko lang po na kg ang gamitin. We do not use pounds in computing meds.

🤓step 2: Check kung aling klase ng paracetamol ang available sa bahay. Look at the attached photo. Andiyan po sa taas ng word na children.

❌ANG ML NA LAMAN NG ISANG BOTE AY WALANG BEARING SA MAGIGING DOSE. IYANG NUMBER SA ILALIM NG BOX SA TABI NG FLAVOR AY HINDI PART NG COMPUTATION. When we ask ilang mg ang paracetamol sa bahay, ang sagot ay HINDI 10 mL or 60 mL.

Sputum Culture?

🤔"Ang ihi may urinalysis and urine culture kapag suspecting UTI. Ang p**p minsan pinapafecalysis or stool culture. How come hindi pinapatest ng mga pedia ang plema kapag may ubo ang bata? Can't we do sputum culture para malaman kung totoong acute bronchitis lang or baka bacterial pneumonia na pala??? Kung may pneumonia na, can't we do sputum culture para malaman natin ang best antibiotics?"

❤️Hindi mahirap hanapin ang sputum culture. Availability is not the issue. Hindi rin po iyan bagong test na konti lang ang may alam.

🖤mahirap kumuha ng sputum sa young children
🖤ISOLATION OF BACTERIA MAY NOT ALWAYS INDICATE INFECTION
- Normal na hindi bacteria-free ang daanan ng hangin at bibig ng mga tao.
ex. nakakuha ng virus sa ibang tao causing acute bronchitis -> pinasputum culture at nasama sa plema ang mga bacteria na "nakatambay" lang naman sa daanan ng hangin at HINDI naman sila ang cause ng ubo ng patient -> mabibigyan pa ng antibiotics na hindi naman makakatulong dahil virus nga ang cause
🖤TYPICALLY DO NOT ACCURATELY REFLECT THE CAUSE OF THE LOWER RESPIRATORY TRACT INFECTION
- Puwedeng totoong may bacterial infection na nga ang patient but wala pa rin tayong assurance na ang tutubong bacteria sa sputum culture ay talagang the bacteria causing the infection of the patient.

🧑🏻‍⚕️Lahat po ng bagay, even tests, ay may risks and benefits na dapat naiintindihan natin. Hindi po dahil available at afford naman ang test ay ibig sabihin dapat na itong gawin.
