🤓Tatapusin ko bukas ang pagbabasa ng 2026 atopic dermatitis guidelines + editing ng reseta ko on AD. Naumpisahan ko na tonight pero inaantok na ako... Iwasan ang pagmemessage sakin outside 8-9 AM. Thanks.

changes so far:
1. problem - may mga new patients na nagwawalk-in na may flare ng AD and partially dahil tinigil ang maintenance

change - in bold capitalized letters, sinulat ko sa upper part ng reseta for flare and sa lower part ng reseta for maintenance
- nababanggit ko naman na maintenance ang cleanser and moisturizers pero I decided tonight na mas magandang clear pa rin sa reseta

2. problem - crowded ang dati kong reseta ng iba't ibang brands ng cleaser and moisturizer tapos plano ko dati checkan ko lang alin doon ang gagamitin ng pasyente -> ending minsan wala naman doon ang gusto ko kaya hindi ko naman magamit ang print

change - inalis all brands

3. problem - wala akong nakatype dati na tamang way ng pagligo at paggamit ng towel

Presyo ng mga Bakuna:
HINDI Nationally Standardized

🤓Ang daming nagtatanungan ng presyo ng *accines sa Threads tapos random people all over the country ang sumasagot.
🤓Magkakalapit ang presyo ng mga pedia in the same area handling the same target market. By area po hindi by country...

1. laman ng bakuna

Kung si pedia x PCV 20 ang gamit, siyempre very likely na mas mahal ang pa*accine ng pneumococcal sa kanya versus a pedia na PCV 13 pa lang ang gamit. Do NOT assume na same same lang lahat ng iniinject namin.

2. brand

Kung si pedia x uses the WHO-prequalified brand ng antirabies na malaki ang minahal and buong bote ang ininject niya, don't expect na same price siya sa clinic y na hindi WHO-prequalified ang bakuna and ibang style ng inject ang ginagawa so a bottle ng gamot will be used for several patients...

3. presyo ng hospital stocks and/or rent

May mga ospital na walang stocks. May mga ospital na lagpas 1 million ang need bayaran para makapagclinic doon. Saan pupulutin ng doctor ang pambawi ng millions na chinarge sa kanya ng ospital kundi sa higher PF.

4. target market

Kung pumunta ka sa ospital ng mayayaman, don't be shocked na mas mahal ang bakuna mo kaysa kay nanay x na ang doctor ay nasa near palengke or nasa very small clinic katabi ng public school etc. Related din ito sa explanation 3 above...

5. qualifications and experience

ex. pedia x ang dami nang nabakunahan at magaan ang kamay vs doctor y na limited ang alam sa pagbabakuna

May mga naencounter na akong maling area ang tinusok, mali ang number of doses, etc. 4 doses ang ininject mura nga per shot pero sa age niya isa na lang naman dapat susmaryosep 💔💔💔

6. others

👨🏻‍⚕Hindi dahil may mas murang vaccines ay overcharged kayo. Take into consideration the factors above. HINDI same same lang all vaccines and all doctors.

🏥HMOs accepted for physical clinic check up and for admissions. I am currently not accepting HMOs for online check up.

If child is sick (ubo sipon, diarrhea, etc. , then sickness po ang ichecheck and NOT included ang well child issues (vitamins, deworming, screening tests, etc), and developmental issues (delayed speech, delayed sitting, etc.). Multiple symptoms of related problem (ubo + sipon) sige lang pero
❌multiple types of check up (symptoms of sakit + development issue) ay no no lalo kung hindi ko regular patient... ❌magsisingit ng obviously unrelated symptoms ay no no also (ex. 2 days ubo sipon at saka sabay na rin ang 2 months rashes na never pinacheck)
❌not accepting newborns less than 1 month old... sa ibang pedia niyo ipa"catch" ang baby during panganganak at sa iba niyo ipacheck before 1 month old
🩺FOLLOW UP ay physical NOT online unless otherwise mentioned during the previous check up. Persistence of old symptoms and/or appearance of new symptoms require physical follow up not online tanong tanong.

ACETYLCYSTEINE (Exflem, Fluimucil, Pneumotyl) : dapat may laman ang tiyan

🤔"Bakit kailangang may laman ang tiyan when giving acetylcysteine?"

🤓Ang oral forms ng acetylcysteine (granules in sachet, syrup, tablet na tinutunaw) ay all given na may laman ang tiyan. May laman ang tiyan means kasabay ng pagkain or immediately after kumain. This is done to decrease side effects sa tiyan like nausea/duwal, tummy discomfort, etc.
❌It is NOT given at least 2 hours away from food. Ang interval na iyan ay for most oral antibiotics and HINDI ibig sabihin ay para sa lahat ng ilalagay sa tiyan. Nilalayo ang oral antibiotics at acetylcysteine dahil may possibility na mainactivate ng acetylcysteine ang oral antibiotics.

1. Remind ko lang na WALA akong clinic sa Casa Medica bukas dahil magnininong ako sa binyag. DO NOT MESSAGE ME❗❗❗May clinic ako sa hapon sa Hiprecision MomMecare Las Piñas.

2. Remind ko lang na instead na 2 PM, around 2:30 PM ako magstart sa Hipre bukas dahil may reception pa after simbahan.

3. Sa mga may pinagawa ako pero hindi niyo ginagawa, kargo de konsensya niyo iyan not mine if something bad happens. I did my part. Do yours.

ex. vaccinations, referrals, tests, etc.

OT. Katatapos edit clinic handout on spacer for asthma inhaler:

1. binawasan ng ibang pics para macompress sa isang page
- feeling ko kasi overwhelming tingnan iyong previous edition na spacer instructions pa lang 2 pages na
2. ginawang bullet form ang info kung kailan lilinisin at kung kailan papalitan ang spacer
3. added "sooner if with signs of damage like cracks or scratches"

👨🏻‍⚕️In short, hindi naman life-changing ang changes sa handout on spacer so you don't need to get a copy. Inedit ko siya for my future patients but kung talagang gusto niyo at ako ang regular pedia niyo, PM lang tomorrow.
