โSILENT PNEUMONIAโ โ bagong term na pinapauso sa tiktok/threads/x
Letโs talk about what a chest X-ray can, and cannot, tell us. ๐ซ
A positive-looking chest X-ray does not automatically mean pneumonia, especially when it is interpreted without considering the patientโs actual clinical presentation. Kaya db halos lahat ng xray result sa dulo may nakalagay na โplease correlate clinicallyโ.
Chest X-rays are helpful, but they are not perfect. Interpretation can vary between readers, and the Philippine Clinical Practice Guidelines for Pediatric Community-Acquired Pneumonia (PCAP) emphasize that CXR findings should ALWAYS be correlated with the patientโs clinical findings. Pneumonia may also be considered clinically even when a chest X-ray is not available.
And what about those โhalak,โ noisy breathing, or rattly sounds?
Hindi lahat ng halak ay pneumonia agad. It can sometimes be from nasal congestion or upper-airway secretions, and similar sounds can occur with viral respiratory illnesses. Even certain lung sounds have limited accuracy for diagnosing pneumonia on their own.
At the same time, a simple cough and colds can sometimes progress to pneumonia, so we should not dismiss symptoms either. Hindi po din napepredict ng doctor ang future, so always observe for any progression of symptoms then followup with your doctor.
The important point is: Pneumonia is a clinical diagnosisโnot simply an X-ray diagnosis. The history, physical examination, vital signs, oxygen saturation, and when appropriate, imaging and other tests all need to be considered together.
So before labeling a child as having โsilent pneumoniaโ based solely on an X-ray or a breathing sound, please let a doctor assess the whole picture. โค๏ธ
Ano naman yung, "WALKING PNEUMONIA"
โWalking pneumoniaโ is a commonly used term, not a specific diagnosis. It generally refers to pneumonia where a child may still be up, walking around, and relatively active despite having an infection.
โDoc, hindi naman mukhang sobrang may sakit si Baby, pero ilang araw na siyang inuuboโฆโ
Sometimes, pneumonia doesn't look like the typical pneumonia we imagine.
One possible cause is Mycoplasma pneumoniae, a bacteria that can cause respiratory infections and pneumonia in children. It is often called โwalking pneumoniaโ because some children remain relatively active and do not appear very sick despite having pneumonia.
๐ง Who commonly gets it?
Mycoplasma infection is particularly common among school-aged children and adolescents, although younger children can also become infected.
๐ค What can it look like?
Symptoms can develop gradually, rather than suddenly:
โข Persistent or worsening cough
โข Fever
โข Sore throat
โข Headache or feeling tired
โข Runny nose or other respiratory symptoms
โข Sometimes wheezing
โข In younger children, vomiting or diarrhea may also occur
The cough may persist even when the child otherwise seems relatively okay.
๐ Is every โwalking pneumoniaโ caused by Mycoplasma?
No. โWalking pneumoniaโ is a descriptive term, not a specific diagnosis.
A child who is still playing despite having a cough can still have pneumonia, and pneumonia can also be caused by viruses or other bacteria.
In fact, the Philippine PCAP guideline notes that the clinical presentation of atypical pneumonia can overlap with viral pneumonia.
That is why the child's history and physical examination are important in deciding whether further evaluation or treatment is needed.
๐ Does Mycoplasma need antibiotics?
If Mycoplasma pneumonia is strongly suspected, yes, an appropriate antibiotic may be considered.
But this does NOT mean that every child with cough should automatically receive antibiotics.
This is important because unnecessary antibiotic use can contribute to antibiotic resistance and expose children to unnecessary medication.
๐จ When should parents seek medical attention?
Please have your child assessed if there is:
โ ๏ธ Difficulty or fast breathing
โ ๏ธ Chest indrawing/retractions
โ ๏ธ Bluish lips or face
โ ๏ธ Persistent or worsening fever
โ ๏ธ Child becoming unusually sleepy, weak, or difficult to wake
โ ๏ธ Poor drinking or signs of dehydration
โ ๏ธ Persistent cough with worsening symptoms
โ ๏ธ Any concern that your child is getting worse rather than better
If your child has a persistent cough, let your pediatrician assess the whole clinical picture rather than treating based on the cough alone.
REMEMBER: Hindi lahat ng nakikita online is accurateโand not every piece of information applies to every child.
Social media can help us become more informed parents, but it should not replace proper medical assessment. When in doubt, ask your pediatrician and check reliable, evidence-based sources. ๐ฉบโค๏ธ
