Great News! We are now an official PHILHEALTH YAKAP PROVIDER!
✅ PHILHEALTH YAKAP is an expanded outpatient benefit package that provides primary care services to Philhealth members and their dependents.
✅ Registered Philhealth members and their dependents are eligible.
✅ Be a registered YAKAP beneficiary at Everserve Medical Clinic through the following:
- eGOVPH app
- Philhealth Member Portal
- Local Philhealth Office
Once registered, set an appointment through call/text for your FPE (First Patient Encounter). FPE is a required step to avail free services.

🕛 Clinic Schedule:
Monday- Saturdays from 8:00 am - 5:00pm
No xray and doctor on Saturdays
✅ What You Are Entitled To:
- Consultations with your YAKAP physician
- Basic laboratory tests, if your physician determines they are medically necessary
- Medicines covered by YAKAP, if your physician determines they are medically necessary.
✅ Free lab tests:
CBC, UA, S/E, Sputum Microscopy, FOBT, Papsmear, HBA1C, Lipid Profile, FBS, OGTT, Creatinine,ECG12L, CHEST XRAY PA
✅ Free medicines:
Amoxicillin, Ciprofloxacin, Clarithromycin, Co-amoxiclav, Co-trimoxazole, Nitrofurantoin, Simvastatin, Aspirin, Chlorphenamine, ORS, Paracetamol, Fluticasone+ salmetrol, Prednisone, Salbutamol, Amlodipine, Enalapril, Hydrochlorothiazide, Losartan, Metoprolol, Metflormin, Gliclazide
✅ Your Co-payment
Private YAKAP providers are authorized to collect a maximum PHP 900 per year only. After that, all covered YAKAP services are free for the rest of the year.
✅ One-Year Agreement
It is a one-year care relationship. You cannot transfer to another YAKAP clinic mid-year. Requests to transfer must be submitted in October–December and it will take effect in January of the following year.
