Macrocyst of the Breast / 50/F / (+) family hx of breast cancer

50/F; with ultrasound breasts, multiple cysts (8), left; 1° diagnosis: fibrocystic changes with 5-cm MACROCYST, left (98% certainty); recommended needle aspiration; would think it over first; came back the next week, aspirated 26 cc of serous fluid with complete disappearance of mass; advised monitoring; forewarned MAY RECUR; recurred 8 months after – aspirated 3 cc; recurred 2 years after – aspirated 7 cc. [then lost to follow-up]

MACROCYST – big cyst (containing fluid) palpable vs MICROCYST – small cyst, not palpable;
MACROCYST - BENIGN / NOT CANCER; MAY RECUR; WILL EVENTUALLY RESOLVED

Family history of breast cancer will not alter the diagnosis of MACROCYST as this was based on physical examination finding of cystic mass and ultrasound finding of cysts. Needle aspiration showed cyst also.

Ultrasound of the Breast - Is it always needed to examine the breasts
Here is a common message to me as a breast specialist;
"Hello doc. Pwede po ba humingi ng referral niyo po for breast ultrasound para lang po makapagpacheck up po ako sainyo. Year 2018 I underwent po ako ng breast mass excision, fibroadenoma leaft breast. Gusto po sana magpacheck ulit. Salamat po doc.
May clinic pdin po ba kayo sa MDH po?"

My answer to the usual request for breast ultrasound by laypeople:
"I suggest you see me first - baka hindi kailangan mag ultrasound - examinin muna kita - if there is a need for ultrasound , then I will request for it."
I recommend this approach - examine the breast first through inspection and palpation by a breast specialist before deciding to order for ultrasound. The ultrasound may not be needed after physical examination of the breast. The ultrasound of the breasts is usually associated with a lot of false positive readings which may cause undue anxiety and which may lead to unnecessary operation like biopsy. Physical examination of the breast by a breast specialist carries a 98% to 99% accuracy rate. If no dominant breast mass is discovered on physical examination, ultrasound is not needed anymore. Male breast cancers are rare.
I have encountered probably only 2 to 3 cases in my 44 years of practice (1982 to 2026).
Lately, June 2026, I personally encountered another case – patient of my son – whom we co-managed.

June 2026

Recently encountered a case of a 58-year-old Filipino male with breast cancer located outside the periphery of the subareolar area, specifically, in the right upper quadrant, several centimeters below the clavicle. Initially excised under local anesthesia with a preop diagnosis of epidermal cyst. Histopath revealed invasive breast carcinoma. Breast panel done positive for ERA and PRA.

Patient underwent sentinel node biopsy, right. Two nodes lighted up – tested – positive for tumor. An axillary dissection was done with no other lymph nodes tested positive. Right Total mastectomy specimen (in the ni**le areolar area) was done was negative for tumor. The previous site of excision just below the clavicle showed no residual tumor.

The area oriented and designated as breast connection between the subareolar tissues and the site of the previous excision did not showed breast tissue did not show breast connection between the two sites. Thus, the previously excised breast cancer just below the clavicle was considered as arising from an ectopic breast tissue.

FIBROADENOMA of the Breast

Fibroadenoma is a common benign (non-cancerous) breast tumor, usually occurring in adolescents and young women, but it can occur at any age.

Management depends on the patient’s age, symptoms, size of the lesion, imaging findings, and biopsy results.
Once a breast lump has been confirmed as a fibroadenoma by appropriate clinical, imaging, and/or biopsy evaluation,
the usual management is

OBSERVATION AND REASSURANCE, with surgery reserved for lesions that are growing, symptomatic, cosmetically deforming, suspicious, or desired by the patient for peace of mind.

Started in 1982 – Clinic across Philippine General Hospital; then Medical Center Manila (ManilaMed); then MANILA DOCTORS HOSPITAL (up to now – 2026 = current ROJoson Breast Wellness Clinic) - where I have at least 200 cancer patients in remission for more than 10 years as of May 30, 2026.
