Ear Discharge (Otorrhea): Causes, Signs & Management
Ear discharge means fluid draining from the ear canal. It may be watery, pus-like, bloody, or foul-smelling, and the cause can range from a minor outer-ear infection to a perforated eardrum or chronic middle-ear disease.
🟣 Ear Discharge
➟ Fluid may be clear, yellow, green, bloody, or foul-smelling
➟ The appearance of the discharge can provide clues, but an ear examination is usually needed to identify the cause
🟣 Ear Pain
➟ Pain or tenderness is common with infection or inflammation
➟ Pain when pulling the outer ear or pressing the tragus is more suggestive of otitis externa (swimmer’s ear)
🟣 Reduced Hearing
➟ Swelling, fluid, wax, or discharge can temporarily block sound
➟ Middle-ear infection or an eardrum perforation can also reduce hearing
🟣 Itching
➟ Itching is especially common in outer-ear infections
➟ Eczema, dermatitis, or irritation of the ear canal can also cause itching and discharge
🟣 Outer-Ear Infection
➟ Otitis externa affects the ear canal and may cause pain, redness, swelling, itching, and discharge
➟ It is often associated with moisture, swimming, ear trauma, or inserting objects into the ear
🟣 Middle-Ear Infection
➟ A middle-ear infection can sometimes cause the eardrum to perforate
➟ Pus or fluid may then drain through the ear canal
➟ Ear pain may suddenly improve after the eardrum perforates because pressure is released
🟣 Other Possible Causes
➟ Eardrum perforation after trauma or pressure changes
➟ Foreign object in the ear canal
➟ Ear-canal eczema or chronic skin disease
➟ Chronic middle-ear infection
➟ More rarely, conditions such as cholesteatoma can cause persistent, often foul-smelling discharge and hearing loss
🟣 Management
➟ Keep the affected ear dry
➟ Do not insert cotton buds, fingers, or other objects into the ear
➟ Treatment depends on whether the problem comes from the ear canal, eardrum, or middle ear
➟ Antibiotic or other ear drops may be prescribed when appropriate
➟ Some ear drops should not be used if the eardrum may be perforated, so avoid self-treating with leftover drops
➟ Suitable pain relief may be used if needed
🟣 Seek Medical Care If
➟ Discharge lasts more than a few days or keeps returning
➟ The discharge is bloody, foul-smelling, or accompanied by significant hearing loss
➟ There is severe pain, fever, dizziness, or swelling around the ear
➟ You have diabetes or a weakened immune system and develop severe ear pain or discharge
➟ Clear watery fluid appears after a significant head injury — this requires urgent emergency assessment
