Early Detection of Liver Cancer Using the GALAD Score
The GALAD score is a validated blood-based model used to support the early detection of hepatocellular carcinoma (HCC), the most common primary liver cancer. It combines patient demographic factors—gender and age—with three serum biomarkers: alpha-fetoprotein (AFP), AFP-L3%, and des-gamma-carboxy prothrombin (DCP or PIVKA-II).
By integrating these variables, the GALAD score improves the identification of HCC, particularly in patients with chronic liver disease or cirrhosis who are at high risk of developing liver cancer. Studies have shown that the GALAD score can detect early-stage HCC with higher sensitivity than AFP alone, including tumors that may not yet be visible on routine imaging.
As a non-invasive blood test, the GALAD score may serve as a useful adjunct to ultrasound surveillance, helping clinicians identify patients who require further diagnostic evaluation. Early detection is critical because HCC identified at an early stage is more likely to be treated with curative options such as surgical resection, liver transplantation, or local ablative therapy.

The UTAS-Wako GALAD Score is a validated diagnostic algorithm used to improve the early detection of hepatocellular carcinoma (HCC), the most common form of primary liver cancer. It combines patient demographics (Gender and Age) with three serum biomarkers—Alpha-fetoprotein (AFP), AFP-L3%, and Des-gamma-carboxy prothrombin (DCP/PIVKA-II)—to generate a risk score for HCC. Compared with the use of AFP alone or conventional imaging surveillance, the GALAD Score has demonstrated higher sensitivity and specificity for detecting early-stage liver cancer, including tumors that may not yet be visible on imaging. The UTAS-Wako platform provides standardized measurement of these biomarkers, making the GALAD Score a valuable adjunct in the surveillance of high-risk patients with chronic liver disease or cirrhosis. It is intended to complement, rather than replace, imaging modalities and clinical evaluation.
