Everyone fears the nerve. Almost nobody talks about the artery.

A perforation of the lingual cortical plate during implant placement can injure the sublingual vascular plexus and rapidly produce a floor-of-mouth hematoma. The tongue is pushed superiorly and posteriorly, and the airway—not the bleeding—becomes the immediate concern.

⚠️ Recognize it:
• Rapidly rising floor of the mouth
• Tongue displaced up and back
• Voice change or dysphagia
• Bleeding that does not respond to local pressure

⏱️ Remember: the hematoma may develop during surgery or several hours after the patient has already left the office.

🚑 Management priorities:

1. Secure the airway
2. Activate EMS
3. Transfer the patient to the hospital, even if the bleeding appears to slow or stop.

A clinical trial published in the European Heart Journal found that treating severe gum disease may do more than protect your teeth—it may help slow changes in blood vessels linked to heart attacks and strokes.

Researchers followed patients for 2 years and observed that intensive gum treatment reduced inflammation, limited bacteria entering the bloodstream, and slowed the thickening of the carotid arteries—the major vessels supplying blood to the brain. Even small reductions in this thickness are associated with meaningful improvements in cardiovascular risk.

This doesn’t mean dental treatment replaces heart care. But it highlights something critical:
