We are now accepting new appointments via QR code (Google Form). Since we are experiencing an overload of inquiries for appointments, we are opening this form to ONLY 20 REGISTRANTS PER DAY, open during Mondays - Fridays, 8am -12nn (closed during weekends and holidays)
