PRE-REGISTRATION FOR PHILHEALTH

REQUIREMENTS
1.PMRF FORM (pumunta ng NGC Health Center para sa Form)
2.Xerox ID with 3 signature (applicant)
3.Birth Certificate (if minor and Don't have a Valid ID)
4.Xerox Valid ID of parents with 3 signature (if minor)
5.Submit the form and Requirements to NGC Super Health Center until September 14, 2026

Sept 14 | 8am onwards at NGC health center.

✅HPV DNA testing/papsmear (women 30-65yrs old)
✅Breast exam
✅IUD Check up
✅IUD insertion
✅Implant insertion
✅Implant removal (expired only)
..and other Family planning methods.

Magandang araw po sa lahat.
Amin pong ipinababatid na simula sept 1 ang NGC Health Center ay bumalik na po sa dati at orihinal nitong lokasyon sa MRB gate 1, katabi ng 7/11 sa likod po ng NGC lying in.

Ang ating clinic hours po ay Monday to friday 8:00-5:00pm.
