She came back—not because it failed, but because it worked.

Almost a year ago, our patient traveled from Texas, USA 🇺🇸to the Philippines 🇵🇭 for her first FAMI™ facial fat grafting.

At the time, she specifically asked us to be conservative. Her thinking was simple: “We can always add more later.”

And I agreed. With facial fat grafting, we don’t need to chase maximum volume in one session. The goal is to restore what has been lost while preserving the patient’s natural facial identity.

Almost a year later, she returned—even after losing 2 kg of body weight. The improvement from her first FAMI™ was still evident.

This time, she told us she wanted more—and left the decision of how much fat to place and where to us.

That, for me, is one of the strongest endorsements of a procedure.

Proof of concept for one of the most misunderstood truths about fat grafting: it’s not just volume, it’s regeneration.

Her story started with too much, too fast — multiple energy-based device sessions (think ulthera, HIFU, jaw tox) done all at once, without regard for how much facial fat she’d lose in the process. The result: sunken, haggard, prematurely aged.

She looked for a better technique of facial fat grafting — and that search led her to FAMI® (Fat Autograft Muscle Injection), and to us.

What these photos show, 4 years and 5 months later, is more than restored fullness. We performed the procedure once — no special skincare regimen after. By her own account, her acne breakouts improved by more than 90%, and she seldom breaks out anymore. The Art of Fat Grafting

This patient’s volume loss came from Parry-Romberg syndrome, also known as progressive hemifacial atrophy — a rare condition, and rarer still in male patients, that progressively breaks down facial fat, and sometimes muscle and bone, often continuing to advance over time. It’s a far less forgiving canvas than ordinary age-related volume loss.

Over the course of 7 years and 9 months, FAMI™ was performed six times to address the progression of the disease. These photos were taken 6 months after the final session.

Fat grafting did two things here: it restored the volume the disease had taken, and — over time — visibly improved the skin’s texture and pigmentation. No further correction has been needed since.

Results in Parry-Romberg syndrome vary by patient, largely depending on how far the disease has progressed and how active it remains at the time of treatment.

If a result can hold against a disease actively working against it, it says something about what’s possible when volume loss is simply age, not pathology.

What if we’ve been building the pyramid upside down?

Most facial rejuvenation talks start the same way: combination treatments — facelift, fat grafting, peels, lasers— recommended as early as 40, stacked together as “comprehensive rejuvenation.”

But stack enough procedures on a face and you’re not treating the problem. You’re compensating for what each individual step doesn’t fix.

Because most aging faces have two problems, not one: volume loss, and skin quality loss. A facelift repositions tissue — it doesn’t replace volume, and it doesn’t rebuild skin.

So why does surgery come first?

The reconstructive ladder taught us to start with the least invasive option that solves the actual deficit. Somewhere in facial rejuvenation, we stopped applying that logic.

Base of the pyramid: restore volume where it’s missing — FAMI fat grafting, anatomically placed. Then treat the skin itself — nanofat delivered via Dermapen 4. Regenerative signal, no incision.

Solve volume and skin quality first. Surgery becomes the exception, not the default — reserved for true excess skin or ptosis that grafting alone won’t resolve.

Full-face fat grafting using the FAMI technique — 3 years and 2 months after a single session. No repeat treatments, no fillers to maintain. Just his own tissue, restored.

What we didn’t expect when we started documenting this case: alongside the facial rejuvenation, his long-standing psoriasis cleared — and has stayed clear ever since. That observation was presented at the 9th ICPAS & ISPRES 2026 Congress in Shanghai, shared with the medical community as a case worth investigating further.
