An update on our 29-year-old patient with longstanding deep acne scars.

After years of multiple treatments, he underwent one session of full-face FAMI™ fat grafting combined with Dermapen 4 and nanofat.

Almost two years later, the improvement remains visible—softer and shallower scars, improved skin texture, and a more even overall appearance.

Two days after FAMI™.

Fat grafting is not simply about filling a hollow. It is about restoring volume, contour, and structural support where they have been lost.

Notice the small mole beneath the lower eyelid in the first photo. Using it as a visual reference, it appears closer to the lid margin after treatment—illustrating the subtle lifting effect that can accompany volume restoration.

The second photo shows the broader change: a smoother lid–cheek transition, restored midface contour, and a more supported appearance—without surgery or sutures.

And at only two days: minimal bruising, minimal swelling.

Because recovery should also be part of a good result.

12 Years Ago Today — ISPRES Miami 2014

Twelve years ago today, I was in Miami attending the 3rd ISPRES Congress, continuing a journey that began at the first ISPRES Congress in Rome in 2012.

My interest in fat grafting and regenerative plastic surgery kept bringing me back — Rome, Berlin, Miami, Beijing, Dubai, Tokyo, and Shanghai.

I started as a delegate, eager to listen, learn, and understand where this exciting field was heading. Years later, I had the privilege of returning to Tokyo as a speaker, and most recently to Shanghai 2026 as a speaker and moderator, sharing my own clinical experience and what I have learned along the way.

Looking at these photos from Miami reminds me that expertise is built over time — through curiosity, experience, observation, and continuous learning.

The science and techniques have evolved tremendously, and there is still much more to discover. But my fascination remains the same:

Fat is more than volume. It is living tissue with remarkable structural, reconstructive, and regenerative potential.

From sitting in the audience to eventually standing at the podium — still learning, still evolving, and grateful to be contributing to a field that has fascinated me for more than two decades.

EPISODE 1 — WHY FAT FIRST?

Sometimes the problem isn’t just the scar you see. It’s what lies underneath it.

This forehead scar resulted from a motorcycle accident in 2021. The patient subsequently underwent two scar revisions. His second revision, performed two years ago, initially healed beautifully—almost a hairline scar at one month.

But over time, it gradually widened, became hypertrophic and depressed.

Instead of proceeding directly to a third scar revision, we chose a different strategy:

Treat the tissue underneath first.

We performed fat grafting beneath and around the scar to improve the underlying tissue and depressed contour before considering another surgical revision.

Now, at Day 8, the patient describes the scar as shallower and “whiter,” and personally feels it has improved by about 50%.

But this is not the final result. The fat graft still needs to settle and the tissues need time to remodel. We plan to reassess over the next 4–6 months before proceeding with scar revision.
