Clinical Note:
Immediate Implant - Mandibular Right 2nd Premolar

Patient: Female, early 50s

Chief Complaint:
Wants tooth extracted and replaced with a dental implant - lower right 2nd premolar

Clinical Findings:
Clinical examination of tooth #45 reveals a fractured crown with caries extending into the pulp chamber.

Radiographic Findings:
Radiographic analysis revealed a periapical abscess associated with tooth #45.

Treatment Plan:
Extraction of tooth #45 and immediate implant placement was discussed and consented.

Procedure:
Under local anesthesia, atraumatic extraction of tooth #45 was performed.
Following standard drilling protocol, a MegaGen Blue Diamond 4.1 x 10mm implant was placed:
Initial insertion torque: 35 Ncm
ISQ: 60 / 54 (buccal/lingual or mesial/distal)

The residual gap was packed with allograft bone material.
Primary closure was achieved with sutures.

Post-Op Plan:
Post-operative instructions given.
Follow-up schedule:
Monitor Osseointegration

Clinical Note:

Immediate Implant - Maxillary Left Canine

Chief Complaint:

Fractured tooth after incomplete root canal treatment

History:

Patient underwent root canal treatment several months ago but was unable to complete the treatment due to logistical reasons.
She presents with a fractured maxillary left canine.

Clinical Findings:

Clinical examination revealed a fractured maxillary left canine, tooth #23, with the fracture extending to the cervical third.
No pain reported on examination.

Radiographic Findings:

Radiographic analysis confirmed the fracture and revealed a periapical abscess.

Treatment Plan:

Patient opted for dental implant treatment. Immediate extraction and implant placement discussed and consented.

Procedure:

Under local anesthesia, immediate extraction of tooth #23 was performed.
Thorough debridement and curettage were performed to ensure no granulation tissue remained.

Following standard drilling protocol, a MegaGen Blue Diamond 3.7 x 13mm implant was placed:
Initial insertion torque: 45 Ncm
ISQ: 70

A healing abutment was placed. The residual gap was packed with allograft bone material.
Primary closure was achieved with sutures.

Post-Op Plan:

Post-operative instructions given. Follow-up scheduled. Monitor for osseointegration.

🇺🇸A historic milestone in global dentistry - mercury-based dental amalgam is set to be completely phased out by 2034, following a landmark decision at the Minamata Convention on Mercury (COP-6) in Geneva.

More than 150 countries have officially agreed to eliminate the use, import, and export of mercury in dental amalgam - marking the end of an era that began over 175 years ago. The decision aims to protect human health and the environment from mercury, which is recognized by the World Health Organization (WHO) as one of the top 10 chemicals of major public health concern - known to harm the nervous, immune, and renal systems and to persist as a pollutant in nature.

The agreement sets a science-based, time-bound plan to transition toward mercury-free dentistry by adopting modern, safer restorative materials such as composite resins, glass ionomers, and ceramics. While some nations pushed for an earlier 2030 ban, consensus was reached for 2034 - balancing access, affordability, and readiness of alternatives worldwide.

This global shift represents not only a victory for environmental health but also a strong signal of progress toward a more sustainable and patient-safe future in dentistry. 🌱🦷

Blue Diamond from Mega'Gen

A decade ago, I performed a root canal treatment on a patient and restored the tooth with a dowel post and PFM crown.

Fast forward to 2026, the tooth fractured in half. After discussing options with the patient, he decided on a dental implant.

The treatment plan involves a partial extraction technique, bone grafting, and laser debridement to remove the radicular cyst.

Based on the CBCT analysis, we'll be using a 4.1 x 11.5 Mega'gen Blue Diamond implant.

After the drilling protocols we managed to achieved a decent 45Ncm as an initial insertion torque.
