ANNOUNCEMENT: PROSTHESIS ASSISTANCE PROGRAM

The Office of the Provincial Persons with Disability Affairs Officer - Province of Antique is now accepting applications for prosthesis assistance for qualified Persons with Disability (PWDs).

This program is open to individuals with above-the-knee or below-the-knee amputations (left or right limb).

REQUIREMENTS:

1.Medical Certificate (RHU or Medical Doctor) - Certification stating the applicant needs a prosthesis
3.Certificate of Indigency from Barangay
4.Photocopy of PWD ID (Back to Back)
5.Case Study / Social Case Study
6.Whole Body Picture (Left, right, back, front)

June 30, 2026
